About this role

Job summary

This is an exciting and unique opportunity to be at the forefront of the delivery of our hospital intervention team at Conquest Hospital, Hastings.

The hospital intervention team (HIT) is a multi disciplinary team based in the gateway wards of the acute hospital. The team consists of nurses, occupational therapists and physiotherapists with cross competencies to assess patients for discharge. The gateway wards include AandE, MAU, CDU as well as others.

The purpose of the senior assistant role is to review and assess patients to ensure they can return home. The post involves conducting social and mobility assessments and completing referrals onto different community services as appropriate for discharge. The successful candidate will be given support and training to have the required competencies to complete these assessments, referrals and issue equipment appropriate to the role.

Applicants must have previous NHS experience and demonstrable experience of assessing patients for discharge, including supporting discharge planning and decision-making within a clinical setting. Applications that do not meet this essential requirement are unlikely to be shortlist.

Due to anticipated high levels of interest, this vacancy may close earlier than the advertised closing date once sufficient applications have been received. We therefore encourage interested candidates to submit their application as soon as possible to avoid disappointment.
