Job summary
We are looking to recruit a registered nurse to join our well established, friendly and very busy practice. The post is a minimum of 16 hours per week over 2 days.
Previous experience of working in a GP Practice would be an advantage but not essential as training and development would be given to the right candidate.
Experience in chronic disease management is desirable. Smear taking, contraception and childhood immunisations is desirable but not essential.
The suitable candidate must be computer literate, organised with good time management, motivated, adaptable and enthusiastic as well as being able to work autonomously. The successful candidate must also be an active team player and flexible to work within a friendly nursing team.
Salary negotiable- depending on experience.
6 weeks (pro-rata)- plus bank holidays.
6 Months probation period.

Take a role in assessing, planning, implementing and evaluating care of patients within the practice population.
Ensure high standards of care are provided to all patients who have contact with the nursing team and take appropriate action when agreed standards are not met. In conjunction with other members of the Primary Care Team, assists in the development of guidelines and protocols for managing practice population. Participates in research and assists with audits/clinical trials within the practice. Liaise with all members of the primary health care team and other agencies (such as local authority social services) in order to assure appropriate care is provided for the practice population.
Post holder will be expected to adhere to the NMC Code of Professional Conduct and has responsibility to familiarise themselves with the Health and Safety at Work Act, ensuring the welfare and safety of staff and patients within the practice environment. The Practice reserves the right after appropriate consultation with you, to vary your duties and responsibilities within the parameters of your grade and occupational competence in order to respond to changes in the needs of the Practice. About us
We are a friendly, dedicated practice with a determination to provide high quality clinical care for our patients, at the same time we are highly committed to looking after our doctors as well. Due to retirement an opportunity has arisen to join our friendly forward thinking practice. We are situated in North East Wales on the Welsh/English border. There are good connections to nearby cities with the historical city of Chester being only 5 miles away. There are also excellent schooling options in the local area. We have a list size of approx 9500 patients and deliver our GMS contract over 3 sites. Our main site is Hawarden Health Centre with branch sites in Saltney and Buckley. Hawarden is located 6.7 miles from Chester and is 4 miles from the English border, we are close to motorway links giving direct access to Liverpool and Manchester
We have a team of ANP, Practice Nurse and HCA. We also have an excellent administration team.

Competence
You are responsible for limiting your actions to those that you feel competent to undertake. If you have any doubts about your competence during the course of your duties you should immediately speak to your Lead Nurse/Lead GP.
Registered Health Professional
All employees of the practice who are required to register with a professional body, to enable them to practice with their profession, are required to comply with their code of conduct and requirements of their professional registration.
Risk Management
It is a standard element of the role and responsibility of all practice staff that they fulfil a proactive role towards the management of risk in all their actions. This entails the risk assessment of all situations, the taking of appropriate actions and reporting of all incidents, near misses and hazards.
Records Management
As an employee of the practice, you are legally responsible for all records that you gather, create or use as part of your work within the practice (including patient health, financial, personal and administrative), whether paper or computer based. All such records are considered public records, and you have a legal duty of confidence to service users (even after the employee has left the practice employment). You should consult yourLead Nurse/Lead GPas to the correct management of records with which you work.
Health & Safety Requirements
All employees of the practice have a statutory duty of care for their own personal safety and that of others who may be affected by their acts or omissions. Employees are required to co-operate with management to enable the practice to meet its own legal duties and to report any hazardous situations or defective equipment.

Person Specification
Qualifications Essential
Registered Adult Nurse with the Nursing and Midwifery Council (NMC) Minimum years clinical experience in primary or community care Recognised Practice Nurse qualification (degree level) Experience in chronic disease management and minor illness Ability to work autonomously within a multidisciplinary team
Desirable
Post-registration professional development in primary care, long-term condition management (e.g., asthma, diabetes), immunisation, cervical screening, etc. Chronic disease management courses or certificates.
