Job summary
Havant and Waterlooville PCN are looking for a Social Prescriber to work 15 hours per week, over 2 days.
Social prescribing empowers people to take control of their health and wellbeing through referral to a non-medical link worker who gives time, focuses on what matters to them and takes a holistic approach, connecting people to community groups and statutory services for practical and emotional support.
Link workers support existing groups to be accessible and sustainable and help people to start new community groups, working collaboratively with all local partners.
Social prescribing can help to strengthen personal and community resilience and reduces health inequalities. It addresses the wider determinants of health, such as debt, poor housing, and physical inactivity, by increasing peoples active involvement with their local communities.
As a Social Prescriber, you will work closely with the GP practices and a variety of established services to provide a range of support for patients and deliver the appropriate level of support at the right time.

Main duties of the job
Provide personalised support to individuals, their families and carers enabling them to take control of their wellbeing, live independently and improve their health outcomes.
The post holder will have the confidence to work with clinical and non-clinical colleagues from Havant and Waterlooville PCN practices, PCN colleagues and colleagues working in secondary care, community health, adult social care and the voluntary sector.
You must be an excellent listener, have experience of building strong relationships with people and be committed to supporting local communities to care for each other.
You should have experience of working positively with people facing complex social and emotional challenges.
You will have great interpersonal skills in supporting people, community groups and local organisations.
Strong administration skills and sound IT knowledge is also required.
Access to own transport and ability to travel across the locality on a regular basis, including to visit people in their own homes is essential.
About us
Havant & Waterlooville Primary Care Network (PCN) has five member GP practices in South-east Hampshire, stretching from Denmead Practice in the North-west to Bosmere Medical Practice to the south of Havant. Together our practices are responsible for 67,000 patients.
Primary Care Networks were established in 2019 to extend the range of staff roles and services available from GP surgeries and, by working across a number of neighbouring practices, to share expertise and resources across a wider area.
Our Social Prescribers work flexibly with patients registered at all five network practices.
Job description Job responsibilities
Provide personalised support to individuals, their families and carers enabling them to take control of their wellbeing, live independently and improve their health outcomes. Accept referrals from GP practices within our Primary Care Network. Offer between 1 and 6 sessions for up to an hour, face-to-face or virtual and offer follow up where needed. Co-produce a personalised support plan to improve health and wellbeing, introducing, connecting, and supporting patients to access community groups and statutory services. Ensure all necessary data and information about patients is recorded accurately and confidentially with awareness of information governance best practice. Help patients identify the wider issues that impact on their health and wellbeing, such as debt, poor housing, being unemployed, loneliness and caring responsibilities. Provide earlier intervention whilst waiting or avoiding the need to wait for triage from other services. Where necessary, physically introduce people to community groups, activities, and statutory services, ensuring they are comfortable. Follow up to ensure they are happy, able to engage, included and receiving good support. Work sensitively with patients to capture key information, enabling tracking of the impact of social prescribing on their health and wellbeing. Encourage patients to provide feedback and to share their stories about the impact of social prescribing on their lives. Manage and prioritise your own caseload, in accordance with the needs, priorities and support required by individuals on the caseload. Work closely with the safeguarding leads to ensure any issues and concerns are escalated and dealt with quickly and effectively. Refer patients back to other health professionals/agencies, when their needs are beyond the scope of the link worker role e.g., when there is a mental health need requiring a qualified practitioner. Build relationships with key staff in GP practices within the local Primary Care Network (PCN), attending relevant meetings, becoming part of the wider network team, giving information and feedback on social prescribing. Organise and help support weekly MDTs (multi-disciplinary team). Be able to work flexibly in-hours and out of hours. Participate in regular clinical supervision with a GP.

Experience Essential
Minimum of 2 years experience of working with healthcare professionals and/or 2 years previous experience in the NHS, social care or relevant field

Experience of working in a multi-disciplinary setting
Qualifications Essential
Educated to A level/ NVQ Level 3 or experience of working at this level
Desirable
Social Prescribing Qualification Educated to Diploma level BTEC/HNC/HND or equivalent
