An exciting opportunity has arisen for an enthusiastic and motivated Clinical Pharmacist to join our friendly and supportive general practice team, working across Marisco Medical Practice and Liquorpond Surgery.
This is an excellent opportunity to play a key role in delivering high-quality, patient-centred care while helping to optimise medicines use and improve health outcomes across our two practices. You will work closely with Pharmacists, GPs, nurses, pharmacy technicians, and the wider multidisciplinary team within the practice, across the PCN to provide expert clinical pharmacy services.

Main Responsibilities
As our Clinical Pharmacist, you will:
Undertake structured medication reviews and optimise medicines for patients with long-term conditions. Provide specialist medication advice to clinicians and patients. Support safe, cost-effective prescribing in line with national and local guidance. Manage medicines-related queries from patients and healthcare professionals. Monitor high-risk medicines and undertake clinical audits where appropriate. Manage medication alerts and medication shortages. Support prescribing quality improvement initiatives and medicines optimisation projects. Participate in the management of repeat prescribing systems. Work collaboratively with community pharmacies, secondary care, and other healthcare providers to ensure seamless patient care. Contribute to the delivery of enhanced services and practice development.

JOB SUMMARY

An exciting opportunity has become available for an enthusiastic and forward-thinking Clinical Pharmacist to join our multidisciplinary team, working across both practices. This role will work closely with the Senior Clinical Pharmacist, GPs, Practice Managers, nursing teams, pharmacy technicians, administrative teams and wider community healthcare professionals to optimise medicines management, improve prescribing quality and support excellent patient care.
The post holder will play a key role in delivering safe, effective and evidence-based prescribing, supporting long-term condition management, improving medicines optimisation and ensuring patients receive the best possible outcomes from their medicines.
The role will focus on:
Working as part of a multidisciplinary team to provide expertise in clinical medicines management, undertake structured medication reviews, support long-term condition management, manage medicines following transfer of care, improve prescribing systems and undertake repeat prescription authorisation and acute prescription requests. Ensuring the safe, accurate and timely prescribing of medicines for patients. Supporting patients to achieve the best outcomes from their medicines through education and optimisation. Acting as the principal contact for clinical medication queries from community pharmacies, care homes and other healthcare professionals. Leading the management and implementation of MHRA Drug Safety Alerts, medicine shortages and national safety guidance across both practices. Implementing medication changes where clinically appropriate. Undertaking clinical audit, quality improvement projects and prescribing safety reviews. Supporting medicines optimisation initiatives and quality improvement programmes. Working closely with GPs, practice staff, pharmacy technicians and administrative teams to improve prescribing processes. Supporting compliance with Care Quality Commission (CQC) standards relating to safe and effective care. Developing and reviewing medicines management policies and ensuring prescribing is consistent with local and national guidance. Supporting achievement of Quality and Outcomes Framework (QOF), enhanced services and other contractual requirements. Maintaining professional competence and undertaking continuing professional development. JOB ROLES AND RESPONSIBILITIES
Main Duties
Undertake medicines reconciliation following hospital discharge or other transfers of care and update patient records accordingly. Work collaboratively with patients, carers and community pharmacies to ensure continuity of medicines following discharge. Support achievement of practice Quality and Outcomes Framework (QOF) targets and locally commissioned services. Review and develop repeat prescribing protocols and Standard Operating Procedures. Provide prescribing advice to GPs and clinical staff to improve prescribing safety, quality and cost-effectiveness. Implement prescribing changes in response to MHRA alerts, medicine shortages and national guidance. Maintain accurate clinical records using agreed templates and coding standards. Support compliance with Care Quality Commission (CQC) standards. Optimise medication monitoring systems and clinical recall processes. Lead and participate in prescribing audits and quality improvement initiatives. Assist in monitoring prescribing budgets and liaise with the Integrated Care Board (ICB) Medicines Optimisation Team. Produce evidence-based prescribing guidance and educational resources. Undertake structured medication reviews, provide patient education and undertake home visits where appropriate. Support implementation of medicines management policies including shared care prescribing. Promote the safe storage, handling and use of medicines and recommend improvements where required. Participate in multidisciplinary clinical meetings. Respond professionally to medication-related queries from patients and healthcare professionals. Advise on sourcing medicines and appropriate alternatives during shortages. Identify areas of prescribing risk and develop action plans to improve safety and efficiency. Benchmark prescribing against local and national guidance and make recommendations to improve prescribing quality. Organise and prioritise workload to meet agreed deadlines. Deliver high-quality clinical pharmacy services across both practices. Work collaboratively with GPs, nurses, practice staff and external healthcare professionals to improve patient outcomes. Actively prescribe medication and be registered as an Independent Prescriber on the GPhC register. Support patients and carers to understand and manage their medicines effectively. Develop effective relationships with local health, social care and voluntary sector organisations. Refer patients to appropriate health and wellbeing services where required. Represent the practices at locality or Integrated Care Board meetings when requested. Collect and report prescribing and service data as required. Promote a positive, supportive and patient-focused culture. Undertake any other duties commensurate with the role. As this is an evolving role, this job description is not exhaustive.
