Job summary
We are seeking a highly organised and proactive Triage Coordinator to join our Community Triage & Response Service (CNRS). This is a key non-clinical role that supports the efficient coordination and prioritisation of urgent referrals, ensuring patients receive timely and appropriate care in the community.
As the central point of communication for the service, you will work closely with clinicians, patients, carers and partner organisations to support the safe and effective deployment of the response team. The role is pivotal in maintaining smooth service delivery within a busy, fast-paced environment where priorities can change rapidly.
Whilst this post is advertised as an administrative role, we also welcome applications from clinical staff who feel they have the skills and experience to excel in this coordination-focused position.
Main duties of the job
As the Triage Coordinator, you will be responsible for receiving, processing and coordinating urgent community response (UCR) into the Community Nursing Response Service (CNRS), ensuring they are managed efficiently and in accordance with service processes and operational priorities.
You will support the prioritisation of referrals and act as the central communication point between clinicians, patients, carers and partner organisations, facilitating the effective flow of information across the service.
The role requires you to maintain accurate electronic records and ensure all information is recorded promptly and to a high standard. You will coordinate workloads and support the efficient deployment of clinical response teams, helping to ensure that patients receive timely care. The postholder will also be responsible for escalating concerns, risks or service pressures to the appropriate clinical and operational leads, contributing to the safe and effective running of the service.
In addition, you will monitor service activity, support data collection and reporting requirements, and provide comprehensive administrative support to the CNRS team. Through excellent organisation, communication and attention to detail, you will play a vital role in supporting the delivery of a responsive and high-quality community nursing service.
