Job summary
Cerne Abbas Surgery is looking for a Registered Nurse to join our close-knit, supportive Integrated Nursing Team.
We are looking for someone to join the team for up to 37.5 hours a week, but part-time hours could be negotiated for the right candidate(s).
Ideally we are looking for someone with either practice or community nursing experience, but we are willing to provide training for the right person.
You will be required to work some weekends on a rota basis (approx. 1 in 4): where this is the case you will have time in lieu in the preceding or following week.
Note that although we do not operate via Agenda for Change, this role will broadly reflect AfC Band 5 rates of pay: salary is dependent on skills and experience within this range.
We are unable to provide visa sponsorship so applicants must have the right to work in the UK and must live within a distance that is commutable on a daily basis.

Job summary
To be responsible for the implementation of processes for the effective care and management of patients using evidence-based practice, including community care for elderly and housebound patients.
Working as a member of the practice multidisciplinary team, the post holder will ensure nursing services are delivered effectively to the practice patient population.
You will be responsible for one or more clinical areas such as infection prevention and control, health promotion, chronic disease management, various clinics, as well as actively supporting the Integrated Nursing Team Lead in the reviewing and delivery of clinical policy and procedure.
Cerne Abbas Surgery will ensure that the post holder has access to appropriate clinical supervision and an appropriate named individual in the organisation to provide general advice and support on a day-to-day basis.

Primary key responsibilities
To support the Infection Prevention Control (IPC) lead and to work to the IPC Policy. To support the care management programme for the frail and housebound patients registered at the practice To assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice To carry out and embed health promotion and wellbeing programmes To implement and evaluate individual treatment plans for chronic disease patients that promote health and wellbeing To identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to patients health To work closely with other agencies to support patient care To provide routine nursing care to patients in accordance with clinical based evidence, NICE and the NSF To diagnose and manage acute and chronic conditions To review patient medications to enhance compliance To request pathology services and process pathology results as required To provide guidance to patients in the use of prescribed and over-the-counter medications regarding side effects and dosages (within own scope of practice) To maintain accurate clinical records in conjunction with extant legislation To ensure the appropriate maintenance and use of chronic disease registers To comply with the practice chaperone policy To assist GPs with minor surgery when required To support the team in dealing with clinical emergencies To deliver immunisation and vaccination programmes for adults and children To liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.) To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual) To adhere to and act on all safeguarding matters, in accordance with local and national policies To deliver opportunistic health promotion where appropriate To support the provision of travel medicine services Secondary responsibilities
In addition to the primary responsibilities, a Nurse may be requested to:
Take part in the development and review of audit protocols and process for the practice, effectively utilising the audit cycle Take part in the development and review of, and implement, nursing protocols in conjunction with the Integrated Nursing Team Lead Participate in local initiatives to enhance service delivery and patient care, including those initiated at PCN level Support and participate in shared learning within the practice in order to improve patient care Continually review clinical practices, responding to national policies and initiatives where appropriate Participate in the review of significant and near-miss events applying a structured approach, i.e., root cause analysis (RCA) Engage in the planning and implementation of changes within the nursing team, enhancing the provision of services to patients Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation

Qualifications Essential
Registered Nurse (Nursing and Midwifery Council).
Desirable
Mentor or teaching qualification. Post graduate diploma or degree (Chronic Disease Management). Family planning qualification.
Experience Essential
Experience of working as a nurse. Experience of working autonomously. Experience of infection prevention and control measures.
Desirable
Experience of working as a practice nurse or community nurse. Experience of working in a primary care environment. Experience of chronic disease management. Experience of managing elderly and housebound patients. Experience of quality initiatives, i.e., benchmarking.
Clinical Knowledge and Skills Essential
Wound care/removal of sutures and staples. ECGs. Venepuncture. New patient medicals. Requesting pathology tests and processing the results, advising patients accordingly. Understanding the importance of evidence-based practice. Understand the requirement for PGDs and associated policy. Ability to record accurate clinical notes. Ability to work within own scope of practice and understanding when to refer to GPs. Chaperone procedure . Immunisations (routine, childhood and travel).
Desirable
Women's health (cervical cytology, contraception, etc.). Chronic disease management in respiratory, diabetes, COPD etc. Travel medicine. Spirometry. Broad knowledge of clinical governance.
