About this role

Job summary

This is an exciting opportunity to join a specialist community stroke rehabilitation service, supporting people to regain independence and achieve personal goals following a stroke. Working under the guidance of Occupational Therapists, Physiotherapists and other members of the multidisciplinary team, you will help deliver rehabilitation programmes that promote recovery, maximise function and improve quality of life.

The role would suit someone with experience in healthcare, rehabilitation or support work who is keen to develop therapeutic skills within a community setting. It is also an excellent opportunity for those considering a future career in Occupational Therapy, Physiotherapy, Nursing, Speech and Language Therapy or another Allied Health Profession.

Based primarily in patients' homes and community settings, you will support individuals to work towards personalised rehabilitation goals. Duties may include assisting with mobility, upper limb function, cognitive and communication strategies, daily activities, monitoring progress and carrying out therapy programmes prescribed by qualified therapists.

As a key member of the team, you will help people rebuild confidence, independence and participation in everyday life after stroke. While some personal care may occasionally be required, the primary focus is therapeutic rehabilitation and recovery.

Main duties of the job

To carry out rehabilitation plans as prescribed by therapists

Provide assistance to clinical staff in the assessment and treatment of patients

Provide assistance with activities of daily living as per care plan

Be involved in stockchecking of equipment and some basic administration jobs

Attending meetings and training as needed

To escalate concerns about therapy programmes or deteriorating patients to senior colleagues

To work as part of a multidisciplinary team
