Job summary
Woodland Avenue Surgery is seeking an experienced and motivated Practice Nurse to join our friendly and supportive clinical team for 22-24 hours per week across3 days.
The successful candidate will have experience working within a GP practice setting and be confident in managing long-term conditions independently while delivering high-quality nursing care to our diverse patient population. You will work collaboratively with GPs, nursing team colleagues including healthcare assistants and the wider multidisciplinary team to provide safe, effective and patient-centred care.
This is an excellent opportunity for an experienced Practice Nurse who enjoys working autonomously while being part of a supportive Practice Team.
Main duties of the job
Independently manage patients with long-term conditions including: Diabetes Asthma COPD Hypertension Cardiovascular disease Undertake annual reviews and develop personalised care plans. Request and interpret relevant investigations within scope of practice. Support patients with self-management and lifestyle modification. Perform cervical cytology (smear tests) in accordance with national guidelines. Ensure appropriate documentation and follow-up of results.Childhood and adult immunisations. Travel vaccinations (where trained). Wound care and dressings. Removal of sutures and clips. Ear irrigation (where appropriate). ECG recording. Venepuncture. Blood pressure monitoring. NHS Health Checks. Health promotion and lifestyle advice. Infection prevention and control. Stock management and vaccine cold chain management. Contribution to annual Influenza Vaccination Programme Maintain accurate electronic clinical records using SystmOne. Participate in clinical audits and quality improvement initiatives. Work within NMC Code and practice policies. Recognise and respond appropriately to safeguarding concerns. Participate in mandatory training and professional development. Work closely with GPs, Nursing Team and the wider multidisciplinary team. Support colleagues to deliver excellent patient care. Attendance and contribution at practice meetings and service development.

Woodland Avenue Surgery is a well-established, forward-thinking GP practice serving a diverse community in Luton. We have 5 GP Partners with a patient population of approx. 13,500.
Our Nursing Team consists of a Full time Practice Nurse Lead, X2 HCAs and PCN Nurse supports the Practice 1-2 days a week.

Qualifications Essential
Essential Qualifications Registered Nurse with current NMC registration. General Practice Nurse experience. Cervical Cytology Smear qualification. Evidence of continuing professional development. Experience Previous experience working as a Practice Nurse in a GP surgery essential. Experience managing long-term conditions independently. Experience delivering cervical screening. Experience providing routine treatment room nursing services. Experience using electronic clinical systems preferably SystmOne. Knowledge & Skills Excellent communication and interpersonal skills.
