Job summary

This is an opportunity to make a real difference to the delivery of patient care as we continue to develop a cohesive approach to integrated services across four practices within the PCN.

Working under the strategic guidance of the Clinical Director, alongside the Board Members and supported by the Practice Managers, you will play a pivotal role in driving the strategic development of the Primary Care Network as a key partner within the local neighbourhood health system. The post holder will lead transformational change, neighbourhood development, workforce integration, partnership working, service redesign and financial sustainability, ensuring the PCN is well positioned to deliver excellent patient care and respond to evolving NHS priorities and neighbourhood health reforms.

Main duties of the job

The PCN & Neighbourhood Development Manager is a senior leadership role responsible for driving the strategic development of the Primary Care Network as a key partner within the local neighbourhood health system. The post holder will lead transformational change, neighbourhood development, workforce integration, partnership working, service redesign and financial sustainability, ensuring the PCN is well positioned to deliver excellent patient care and respond to evolving NHS priorities and neighbourhood health reforms.

You will be the first point of contact for the PCN, acting as an enabler to improve services for patients and contribute to the sustainability of general practice. The post holder will ensure that the PCN and its member practices have a strong voice and are empowered to play a key role within the transforming healthcare environment.

Job description Job responsibilities

1. Strategy and Leadership

Lead the development and implementation of the PCN's Neighbourhood Health and Integrated Neighbourhood Team (INT) agenda, ensuring alignment with local Place-Based Partnership and Integrated Care Board priorities and national priorities.

Develop and deliver programmes that support the NHS Long Term Plan, Primary Care Access Recovery Plan, prevention agenda, population health management, and health inequalities priorities. Provide programme leadership across key areas including prevention, personalised care, health inequalities, urgent care, enhanced access, and integrated care delivery.

Lead the design, mobilisation, and delivery of new neighbourhood-based services, ensuring services are integrated, sustainable and focused on improving patient outcomes.

Support the PCN Board in responding to changing NHS policy, neighbourhood health reforms, and integrated care developments.

Lead the production of annual delivery plans, transformation programmes and business cases that support the strategic objectives of the PCN/Neighbourhood.

Identify, secure, and manage external funding opportunities, transformation grants, and innovation programmes on behalf of the PCN.

Develop and establish strong collaborative relationships with partners and stakeholders to complement and shape local strategy in pursuit of PCN interests.

Take an active role in developing and embedding PCN values and reputation as a provider of high-quality services. Coordinate the delivery of all PCN DES requirements and future contractual developments, ensuring compliance and maximising opportunities for the network. Chair, manage and co-ordinate PCN Board meetings. Be responsible for producing regular performance, workforce, quality, and financial reports for the PCN Board. 2. Governance, Standards and Processes

Maintain and monitor robust governance arrangements including the Network Agreement, Data Sharing Agreements, and Information Governance Frameworks.

Develop, implement, and maintain PCN policies and procedures, working with Practice Managers to standardise processes where appropriate.

Use population health management data, business intelligence tools and NHS insights to identify opportunities to improve outcomes, reduce variation and address health inequalities.

Conduct regular reviews with PCN staff and service providers and facilitate discussions regarding service delivery and achievement of KPIs and quality indicators.

Maintain robust programme management, assurance, and performance monitoring arrangements across all PCN activities.

Develop and maintain organisational risk registers, business continuity plans, and assurance frameworks.

Support the PCN Board in ensuring compliance with all relevant NHS contractual, governance and workforce requirements.

Maintain a high level of knowledge of national policy developments relating to Primary Care Networks, Integrated Care Systems and Neighbourhood Health Services.

Ensure compliance with GDPR, Freedom of Information legislation and other relevant statutory and professional requirements.

3. Workforce and Organisational Development

Develop workforce plans that support service delivery, neighbourhood working, and long-term workforce sustainability.

Support the development of a comprehensive workforce strategy, including optimisation and integration of Additional Roles Reimbursement Scheme (ARRS) staff across the network. Lead on recruitment, selection, and induction of new PCN staff. Carry out all aspects of HR responsibilities of PCN and ARRs staff including performance management, disciplinary etc. Support leadership development, succession planning, and staff wellbeing initiatives across the network. Promote multidisciplinary team development and integrated ways of working across primary care and community partners. Develop strong relationships and engage with service providers, ensuring that service level agreements are met and regular reviews are undertaken. Collaborate with member practices to identify workforce challenges and opportunities and develop collaborative solutions. Develop and maintain HR and training policies, terms and conditions and employee handbooks. Nurture and maintain close working relationships with Practice Managers and wider practice teams, develop strong relationships between PCN staff and practice teams.

4. Financial and Contract Management

Be responsible for the effective management of PCN finances working with the PCN Board, member practices and accountants. Provide regular financial reports to the PCN Board and member practices. Manage the PCN bank account and oversee cash flow monitoring and forecasting. Manage invoicing, claims and funding arrangements. Monitor and check the accuracy of contractual submissions and enhanced service returns. Ensure claims are submitted within required timescales. Maintain accurate financial records and facilitate year-end reporting. Ensure optimal utilisation of all PCN funding streams, ARRS allocations, Enhanced Access funding and locally commissioned service income. Develop business cases and investment proposals supporting future neighbourhood development and service transformation. Identify opportunities to improve financial sustainability through innovation, partnership working, and service development. Monitor financial performance against agreed plans and provide recommendations to the Board regarding risks, opportunities, and investment priorities. 5. PCN Engagement and Neighbourhood Partnerships

Be the first point of contact for Practice representatives, Integrated Care Board representatives and wider system partners regarding PCN operations.

Influence, lead, and support the development of excellent relationships across the network and wider stakeholders including health and social care, patients, Healthwatch, the voluntary sector and community organisations. Acting as the lead officer for neighbourhood development, fostering effective partnerships between general practice, community services, social care, mental health providers, voluntary organisations, and local communities. Coordinate and facilitate Integrated Neighbourhood Team meetings and partnership forums. Develop close working relationships with local authority teams, community service providers, mental health services, care homes, housing organisations, and voluntary sector partners. Lead engagement initiatives aimed at reducing health inequalities and improving outcomes for underserved populations. Promote co-production with patients, carers and community groups when developing new services. Build and maintain strong relationships with NHS England, Cheshire and Merseyside Integrated Care Board, Place-Based Partnerships, local authority services, provider organisations, and community partners. Develop and establish a method for effectively capturing patient feedback and using information to help improve service delivery and patient experience. Collaborate with Patient Participation Groups from member practices to help design and improve services. Support the development of neighbourhood-based models of care that enable more joined-up, initiative-taking and preventative care for local populations. 6. In Addition to the Primary Responsibilities, the PCN & Neighbourhood Development Manager may be requested to:

Experience Essential

Experience of successfully developing and implementing projects including establishing working relationships with stakeholders Experience of workforce planning, forecasting and development Experience of managing accounting procedures including budget and cash flow forecasting Experience of identifying and interpreting governmental policy for primary, community and secondary care Experience of HR, performance management, including appraisal writing, staff development, and disciplinary procedures

Desirable

Experience of working at a senior level in a project and/or transformation and/or business management area Strategic understanding and experience of working in the NHS with knowledge and understanding of the roles of the NHS organisation and of the primary care sector Experience of developing primary care provision at scale, for example working with a federation or within a collaboration of practices
