As part of the integrated service provision the post-holder will work in the Therapy Hub supporting the Integrated Multi-disciplinary teams to assist in the prevention of inappropriate hospital admissions, the facilitation of hospital discharge and the provision of home-based treatment in line with the overall objectives of the service.
We are looking for a dynamic, organised, and enthusiastic Rehabilitation Assistant to work closely alongside our Physiotherapists and Occupational Therapists, to support and deliver progressive rehabilitation programmes, aiding our patients to reach their maximum functional abilities and achieve their goals. The role can be physically demanding but very rewarding.
The successful candidate will require good written and verbal communication skills and be computer literate.
The post holder will be supported to work through Band 3 Rehabilitation Assistant Competencies, with training provided as required.
Please note: The split of time between working with the Physiotherapy and Occupational Therapy teams may fluctuate depending on the needs of the patients and service.
Main duties of the job
To assist the Physiotherapists and Occupational Therapists with the assessment and ongoing therapeutic management of community patient. Conditions may include complex permanent disabilities, multiple pathologies, life-limiting conditions, and challenging and unpredictable behaviours.
To assist the Physiotherapists and Occupational Therapists in delivering the Therapy Service, carrying out joint visits with work colleagues within patient's own homes
Reporting back to the Physiotherapists and Occupational Therapists on the outcomes of delegated treatments.
To work as part of the Multi-Disciplinary Team, supporting registered health practitioners, contributing to the effectiveness of team working within service.
Ability to work as a loan worker.

Qualifications Essential
Evidence of literacy / numeracy skills NVQ level II in Health and Social Care or Equivalent.
Experience Essential
Relevant experience in delivering direct patient care in a health or social care setting. Multidisciplinary working
