Job summary
James Alexander Family Practice have an established Care Navigation Team to support & manage patients who contact the practice.
We are currently recruiting for the below fixed term contracts:
1 x 30 hours per week working over 3 days 1 x 20 hours per week working over 2 days Working hours are 8:00am to 18:30pm, Monday to Friday. The specific days worked may vary and will be published in advance as part of a rolling 4-week rota.
The successful candidate(s) will be required to work across two practice sites in line with the rota.
A Care Navigator will be compassionate, motivated, and patient focused. Excellent communication skills and a friendly, approachable nature are essential for this role as you will be the first point of contact for our visitors/patients either face to face or on the telephone. As part of a forward-thinking GP practice, you will play a vital role in supporting patients with complex health needs by ensuring they are guided to the most appropriate care, services, and support at the right time.
This role is central to improving patient access to care, enhancing the patient experience, and promoting overall health and well-being within our practice and the wider community
Main duties of the job
As a Care Navigator, you will be the first point of contact for patients, providing a professional and welcoming service while managing appointments, telephone enquiries, and administrative tasks. You will ensure patient records are maintained accurately, support the smooth day-to-day running of the practice, and signpost patients to the most appropriate services. Working closely with clinical and administrative colleagues, you will prioritise tasks effectively, communicate confidently with patients and external organisations, and contribute to a positive, collaborative team environment. This role requires excellent organisational skills, attention to detail, a patient-focused approach, and the ability to work efficiently in a busy healthcare setting.

James Alexander Family Practice is a large Primary Care Organisation delivering high-quality General Practice services across two sites Bransholme Health Centre and Princes Medical Centre.

The following are the core responsibilities of the Care Navigator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
Process personal, telephone and e-requests for appointments Answer incoming phone calls, transferring calls or dealing with the callers request appropriately Signpost patients to the correct service Process incoming and outgoing mail Initiating contact with and responding to, requests from patients, team members and external agencies Photocopy documentation as required Input data into the patients healthcare records as necessary Scanning of patient related documentation and attaching scanned documents to patients healthcare records Manage all queries (including administrative queries) as necessary in an efficient manner Maintain a clean, tidy, effective working area at all times Support all clinical staff with general tasks as requested.
Person Specification
Qualifications Essential
GCSE grade A to C in English and Maths.
Desirable
Previous experience in a similar role would be advantageous but not essential as full training will be provided.
Skills and competencies Essential
Excellent IT skills, including the ability to use Microsoft office applications including Word, Excel, PowerPoint and Outlook. Excellent communication skills, both written and verbal. Able to adapt to changing workloads and priorities, remaining organised and effective in a fast-paced environment. Proactive and forward thinking.
Desirable
Experience of EMIS Web clinical systems.
Experience Essential
Previous administrative experience. The ability to work individually or as part of a team and to be able to apply yourself in an appropriate manner in a potentially challenging environment.
Desirable
Experience of dealing with sensitive/confidential information. Experience of working in Primary Care.
