As a Home Health LPN, you'll provide skilled nursing care under the direction of the RN Case Manager and attending physician while helping patients safely recover at home. Your responsibilities will include:
• Providing skilled nursing visits in patients' homes
• Monitoring patient conditions and communicating changes to the care team
• Administering medications and treatments
• Educating patients and families on disease management and preventative care
• Helping reduce hospital readmissions through quality patient education
• Collaborating with physicians, case managers, and interdisciplinary team members
• Completing timely documentation in accordance with Medicare and agency guidelines
What We're Looking For
• Current Licensed Practical Nurse (LPN) license
• Minimum of one year of acute care LPN experience (two years preferred)
• Home health experience preferred
• Valid driver's license and active auto insurance
• Strong assessment, communication, and documentation skills
• Compassionate, dependable, and patient-focused
