Job Summary

• Responsible for applying the Nutrition Care Process (NCP) to adult and pediatric populations to identify, treat, and monitor nutrition-related conditions. Provides evidence-based nutrition care that promotes optimal patient outcomes while supporting exceptional customer service and collaborative relationships across the healthcare team. Contributes to the efficiency and effectiveness of patient care through the consistent delivery of high-quality nutrition services.

Essential Functions

• Completes comprehensive nutrition assessments through chart review, patient interviews, anthropometric measurements, and nutrition-focused physical examinations.

• Evaluates assessment findings to identify nutrition risks, nutrition diagnoses, malnutrition indicators, knowledge deficits, and behaviors that may impact health outcomes.

• Develops, implements, monitors, and evaluates individualized nutrition care plans using the Nutrition Care Process.

• Provides medical nutrition therapy, nutrition counseling, education, referrals, and interventions appropriate to patient needs and goals.

• Provides patient and family education using evidence-based approaches that consider health literacy and cultural preferences.

• Monitors patient progress and modifies nutrition interventions to support desired outcomes.

• May perform specialized clinical functions, including feeding tube placement and indirect calorimetry studies, in accordance with organizational policies and competency requirements.

• Documents complete, accurate, and timely patient information within the electronic health record.

• Enters nutrition-related orders in accordance with approved medical staff policies and procedures.

• Participates in interdisciplinary rounds and collaborates with physicians, nurses, case managers, and other healthcare professionals to develop coordinated plans of care.

• Provides nutrition expertise and consultation to clinical and foodservice teams.

• Supports the education and development of dietetic interns, dietetic technicians, registration-eligible dietitians, and new team members.

• Participates in research, quality improvement, performance improvement, and departmental initiatives.

• Maintains timely communication and responds appropriately to electronic communications.

• Performs other duties as assigned.

Knowledge, Skills, and Abilities

• Ability to communicate effectively in English, both verbally and in writing.

• Basic computer proficiency and working knowledge of Microsoft Office applications.

• Comprehensive knowledge of nutrition services, medical nutrition therapy, and evidence-based nutrition practice.

• Knowledge of nutrient analysis software, word processing applications, email systems, and spreadsheet software.

• Ability to research, evaluate, and apply current nutrition science and practice recommendations.

• Ability to serve as a nutrition resource for interdisciplinary healthcare teams.

• Knowledge of quality improvement principles and participation in performance improvement initiatives.

• Ability to effectively participate in interdisciplinary meetings, committee meetings, and patient care rounds.

• Knowledge of regulatory standards and survey readiness requirements.

• Ability to develop, review, and maintain educational and reference materials.

• Ability to identify barriers to patient care and implement process improvement strategies.

• Proficiency in the use of electronic health record systems and clinical documentation practices.

• Ability to analyze assessment data and identify actual or potential nutrition-related problems.

• Knowledge of nutrition diagnosis development, care planning, intervention strategies, and outcome monitoring.

• Ability to identify and address complex discharge planning needs.

• Knowledge of reassessment standards and ongoing evaluation of nutrition care plans.

• Understanding of National Patient Safety Goals, including patient identification and documentation standards.

• Ability to provide age-appropriate assessments, interventions, and education.

• Knowledge of enteral and parenteral nutrition support therapies and the ability to apply recommendations appropriately.

• Strong organizational, customer service, problem-solving, and critical thinking skills.

• Ability to establish and maintain collaborative working relationships with patients, families, physicians, leaders, and team members.

• Ability to work independently while managing multiple priorities and responsibilities.

• Ability to maintain accurate, complete, and timely records.

• Commitment to professional development and staying current with advancements in nutrition and dietetics practice.

Education

• Bachelor's Degree in Nutrition or a related field required.

• Master's Degree in Nutrition or a related field preferred.

Work Experience

• Three years of clinical nutrition experience required.

• Completion of a didactic program with a strong focus in clinical nutrition preferred.

Licenses and Certifications

• Registered Dietitian Nutritionist (RDN) required.

Pay Range:
