• Provide bereavement support and grief education for individuals who have lost a loved one through individual and/or group sessions. • Provide ongoing communication and support to the patient’s primary caregiver for up to 13 months following the patient’s death, in accordance with the bereavement plan of care. • Plan, coordinate and facilitate grief support groups, memorial services, and educational workshops. • Coordinate and monitor volunteer outreach to bereaved families, ensuring that all contacts are documented accurately and included in the client’s bereavement record. • Participate in interdisciplinary team meetings to provide input regarding bereavement risk and family dynamics while collaborating with the nurses, social workers, chaplains and other team members. • Maintain timely, accurate, and compliant documentation of bereavement assessments, grief support sessions, grief groups and all other follow-ups. • Maintain awareness of community bereavement resources to connect families with long-term support beyond hospice care. • Maintain knowledge of bereavement practices, theories of grief and hospice philosophy through ongoing professional development and training. • Other job duties as assigned. Knowledge, Skills, and Abilities:
• Knowledge of counseling skills in the performance of duties in compliance with the legal and ethical parameters established and based on the level of education and certification attained by the employee. [Required]

Education:
• Master's [Required]
Field of Study:
• Clinical Social Work, Marriage and Family Therapy, Mental Health Counseling or related field N/A
Work Experience:
• 3+ years experience in counseling/social work or bereavement [Required]
• Hospice experience [Preferred]

Pay Range:
