Role OverviewThe Care Plan Specialist will manage patients’ prescription amounts and care plan documents, guiding them through their treatment journey and ensuring seamless, compassionate support. This role requires attention to detail, strong transcription skills, and the ability to coordinate efficiently with medical and administrative teams to enhance the patient experience.

Your ImpactEnsure patient care plans are accurate, up-to-date, and compliant with healthcare standards.Provide patients with personalized guidance and support for their treatment journey.Maintain high-quality documentation that supports both patient safety and operational efficiency.

Core ResponsibilitiesPatient Onboarding, Documentation & Transcription:Conduct initial patient intake assessments, capturing critical health information, symptoms, and patient concerns.Transcribe physician explanations, diagnoses, and treatment plans accurately.Maintain comprehensive, up-to-date medical records in compliance with healthcare regulations.Coordinate with physicians to ensure patient information is correctly documented and communicated.Care Plan Management & Patient Support:Develop and maintain personalized care plans based on physician recommendations.Conduct post-appointment follow-ups to ensure adherence to treatment and address patient concerns.Educate patients on medications, lifestyle modifications, and preventive care measures.Collect and analyze patient feedback to improve the patient experience.Coordination, Compliance & Quality Assurance:Collaborate with medical and administrative teams to ensure seamless patient care.Handle insurance documentation, prior authorizations, and billing inquiries.Update care plan documents regularly to reflect changes in treatment plans or patient health status.Liaise with insurance and billing teams to assist patients with coverage and claims support.Requirements

Key Requirements:

2–3 years of experience in patient navigation, healthcare coordination, or medical records management.Strong understanding of medical terminology and patient care processes.Familiarity with care plan documentation, transcription, and healthcare compliance.Attention to detail and ability to manage rapidly evolving processes.Strong communication and collaboration skills for coordinating with medical and administrative staff.Must-Haves (Required):

Healthcare coordination or patient advocacy experienceTranscription and documentation skillsMedical terminology knowledgeExperience managing care plans and patient recordsNice-to-Haves (Preferred):

Pharmacy backgroundFamiliarity with EMR/EHR systemsExperience with patient feedback analysisPrior experience in a medical administrative or operational role
