The Advanced Clinical Practitioner (ACP) will play a pivotal role within Tieve Tara Medical Centre, providing expert, autonomous, patient-centred clinical care to individuals presenting with acute, chronic and complex health needs. Working within their advanced scope of practice, the post holder will undertake comprehensive clinical assessment, diagnosis, treatment, prescribing and ongoing management of patients of all ages, ensuring safe, timely and evidence-based care.

Tieve Tara Medical Centre is a unique, long-established and friendly GP practice located in Castleford, serving a community of approximately 5,000 patients. Working alongside five neighbouring practices as part of the Wakefield North Primary Care Network (PCN), the practice is committed to delivering integrated, person-centred healthcare that meets the needs of its local population.

Main duties of the job

As an experienced autonomous practitioner, the ACP will manage a diverse caseload including minor illness, long-term conditions, preventative healthcare and health promotion, while working collaboratively with GPs, nurses and the wider multidisciplinary team to deliver high-quality primary care services. The post holder will contribute to improving access to care by ensuring patients receive the right care, at the right time, from the right professional, making effective use of practice and wider NHS resources.

Person Specification

Education and Training Essential

Nurse, paramedic or other registered healthcare professional, with current registration with the appropriate professional body (NC, HCPC or GPhC), with no restrictions on practice or pending Fitness to Practice concerns Completed ACP Masters Pathway Full UK Driving License Independent Prescriber Able to work independently without direct supervision

Knowledge Essential

IT competency, particularly in Windows and Office suite Knowledge of NICE Guidelines Understand legal and ethical issues relating to clinical practice Knowledge of own professional accountability and autonomous practice Role of the Care Quality Commission and NHS standards Understands the Advanced Practice Framework and four pillars Understands funding model for primary care and Primary Care networks, including the Quality and Outcomes Framework Understand the PSIRF framework

Skills Essential

Minimum three years post-registration experience (Check with site) Advanced level of clinical practice within current scope of practice Able to understand and analyse complex issues and balance competing priorities to make difficult decisions Understanding of NHS principles and values

Experience of working in a Primary Care setting
