About this role

Job summary

We are looking for an experienced and compassionate Ward Sister/Charge Nurse to lead our friendly and supportive team in managing a 16 bedded rehabilitation ward within Totnes Hospital. This is an exciting opportunity for a highly skilled registered nurse looking to take the next step in their leadership and clinical career.

You will play a key role in supporting patients recovery and promoting independence with a Home First Approach, working closely with our community services to facilitate a safe discharge and prevent hospital readmission.

Main duties of the job

To manage ward effectively and safely, dealing with the daily operational pressures. To ensure high standards of care are given in relation to agreed standards, policies and procedures of Torbay and South Devon NHS Foundation Trust. To facilitate a learning environment to ensure all staff on the ward have the appropriate skills/knowledge and support, identified by regular staff appraisal and annual personal development plan. To foster an environment that focuses on maximising patients independence where possible and utilises a wide range of meaningful activity to support this, including creating a safe space for patients to freely move around the ward. To be proactive with complex discharges, ensuring they are safe and that discharge is at the earliest opportunity.

Care to join us - Dart Ward is a 16 bedded rehabilitation ward within Totnes Community Hospital. Medical Care on the ward is provided by a Drs who work closely with the MDT to ensure safe and timely care with the the aim of supporting patients to live as independently as possible.

Person Specification

Qualifications and Training Essential

Current NMC Registration BSc Hons - Nursing /Healthcare or equivalent experience

Desirable

Leadership or Management qualification Previously demonstrable experience at Band 6/7

Knowledge and experience Essential

Significant Post Registration experience, including leadership/management Evidence of continuing Professional Development Strong Communication and organisational skills

Desirable

Good knowledge of NHS policies and framework

Specific skills Essential

Evidence of implementing change in clinical practice Able to demonstrate research-based practice Evidence of budget management Participation in nursing audit IV cannulation, venepuncture and drug administration Ability to manage change and lead a team effectively
