Job summary
Join our Reablement Team as a Community Reablement Assistant (Level 2) and support adults over 18 in Hampshire needing extra care. Our goal is to maximise their independence and delay or avoid ongoing care, helping them stay at home. Our team, including Community Reablement Assistants, Occupational Therapists and Sensory Officers, collaborate with Health colleagues to support those with short or long-term conditions and those transitioning from hospital.
Main duties of the job
Provide Person-Centred Care: Conduct initial home visits and independently perform reablement interventions to restore independence including intimate personal care, continence care, and moving and handling. Deliver Outcomes: Perform risk assessments, set goals, update reablement plans for Level 1 Assistants and monitor service user progress. Support Operational Delivery:Coach and mentor Community Reablement Assistants and assist Team Leaders, Occupational Therapists and Sensory Officers with activities.
