Heath Lane Medical Centre is seeking to appoint highly motivated and professional Practice Nurse to join our established clinical team.
This appointment forms an important part of our continued service development and future workforce planning. We are looking to recruit a skilled and dedicated nurse who will play an integral role in the future of our nursing team and help us maintain the high standards of care that our patients have come to expect. The successful candidate will join an experienced and supportive clinical workforce and will have the opportunity to develop their skills and areas of interest whilst contributing to the ongoing delivery and development of nursing services within the practice. Main duties of the job
The postholder will be responsible for providing evidence-based nursing care and delivering a range of clinical services in accordance with practice policies, national guidance and professional standards. Responsibilities will include:
Management and review of patients with long-term conditions to include asthma & COPD. Cervical screening and women's health services. Administration of immunisations and vaccinations. Wound care and dressings. Health promotion and preventative healthcare. Supporting patients to manage their health and wellbeing. Maintaining accurate and contemporaneous clinical records. Working collaboratively with GPs and the wider multidisciplinary team. Participating in service development and quality improvement initiatives. The exact scope of the role will reflect the successful candidate's qualifications, experience and areas of clinical interest but will cover the majority of those mentioned above.
The successful candidate will be encouraged to work autonomously within their scope of practice whilst being fully supported by an experienced clinical and management team.

Job Purpose
The Practice Nurse will provide high-quality, evidence-based nursing care to the practice population, working as part of the multidisciplinary team to promote health, prevent illness and manage long-term conditions.
The postholder will deliver a broad range of nursing services within their scope of practice and contribute to the continued development of nursing services across the practice. As part of the practice's workforce planning and succession strategy, the postholder will play an important role in supporting the future delivery of high-quality patient care and maintaining the practice's strong clinical performance and patient outcomes.
The postholder will be expected to work autonomously whilst recognising the need for collaboration and referral to other members of the healthcare team where appropriate.
Main Duties and Responsibilities
Clinical Responsibilities
The postholder will:
Deliver high-quality nursing care within own scope of professional practice. Assess, plan, implement and evaluate patient care. Undertake routine nursing procedures and treatments. Deliver vaccination and immunisation programmes in accordance with national guidance and Patient Group Directions where applicable. Provide wound care, dressings and management of minor injuries as appropriate. Undertake cervical screening in accordance with local and national guidance where appropriately trained. Carry out health checks and health promotion activities. Support patients in managing their own health and wellbeing. Promote healthy lifestyles and preventative healthcare. Monitor and manage patients with long-term conditions, including carrying out annual reviews. Identify and manage clinical risk within own area of responsibility. Refer patients to other healthcare professionals and agencies as appropriate. Long-Term Condition Management
The postholder may be required to:
Undertake reviews for patients with chronic diseases including diabetes, asthma, COPD and cardiovascular disease. Participate in the development and delivery of care plans. Support achievement of Quality and Outcomes Framework (QOF) indicators and any future, similar activity. Assist with clinical audits and quality improvement initiatives. Contribute to improving patient outcomes through evidence-based interventions. Patient Care and Communication

Develop and maintain effective relationships with patients, carers and colleagues. Communicate sensitively and professionally with patients and relatives. Provide education and support to enable patients to make informed decisions about their healthcare. Ensure patient confidentiality is maintained at all times. The postholder will:
Work collaboratively with GPs, Practice Nurses, Healthcare Assistants and the wider multidisciplinary team. Liaise with community nursing services, secondary care providers and external agencies as required. Participate in team meetings and clinical discussions. Maintain NMC registration and work in accordance with the NMC Code. Practise within the limits of their professional competence. Maintain accurate, contemporaneous and legally compliant clinical records. Comply with practice policies, protocols and procedures. Participate in clinical audit, quality improvement and service evaluation activities. Maintain awareness of current clinical guidance, legislation and best practice.
