About this role

Job summary

We are looking for an experienced and motivated Senior Practice Nurse to join our team at Merchiston Surgery for 24 hours per week.

This is a varied senior nursing role combining long-term condition management, general practice nursing and infection prevention and control responsibilities. Working autonomously within your competence, you will provide high-quality patient care while supporting the Nurse Manager, the wider nursing team and our practice priorities.

You will work collaboratively with GPs, practice colleagues and external multidisciplinary teams, using sound clinical judgement and a holistic approach to help patients manage their health and achieve the best possible outcomes.

Main duties of the job

Key responsibilities

Assess and manage patients with long-term conditions, including diabetes, asthma and COPD, in line with NICE guidance and the local formulary.

Plan and deliver clinics by telephone, video or face to face, including domiciliary or care-home visits where required.

Provide a broad range of treatment-room services, including phlebotomy, cervical screening, wound care, injections, immunisations, Doppler assessments and travel-health consultations, according to competence.

Support adult and childhood vaccination programmes, including seasonal influenza and COVID-19 campaigns.

Lead and maintain infection prevention and control standards, undertake audits and work with colleagues and cleaning providers to implement improvements.

Maintain accurate clinical records, use appropriate SNOMED coding and contribute to QOF and other practice targets.

Support quality improvement, audit, clinical risk management and the evaluation and development of nursing services.

Contribute to the education, supervision and professional development of the nursing team and wider organisation.

The postholder will deliver safe and effective nursing care to patients in accordance with the Nursing and Midwifery Council (NMC) Code, evidence-based guidance and practice protocols. As a senior member of the nursing team, the postholder will support the Nurse Manager in coordinating nursing resources and will be accountable for implementing national infection prevention and control standards within the practice.

The clinical focus of the role is the assessment and management of patients with long-term conditions alongside the delivery of general practice nursing services. Working as an autonomous practitioner, the postholder will demonstrate critical thinking and sound clinical decision-making, working collaboratively with the wider practice and multidisciplinary teams to meet patients needs, support practice priorities and develop colleagues.

About you

You will be an experienced practice nurse who is confident working both autonomously and collaboratively. You will bring sound clinical judgement, strong organisational skills and a compassionate, patient-centred approach.

Registered General Nurse with current NMC registration and evidence of meeting revalidation requirements.

Experience of working in primary care and as a practice nurse.

Current competence in cervical screening and inclusion on the smear-taker register.

Current competence in adult and childhood immunisation, supported by appropriate training.

Experience of diabetes management, including part 2 reviews and oral and injectable therapies.

Competence in core treatment-room skills including phlebotomy, wound care, compression bandaging, suture or clip removal, Doppler assessment and travel health.

A clear understanding of Patient Group Directions and Patient Specific Directions.

Excellent communication, interpersonal, time-management and problem-solving skills.

A flexible, supportive and professional approach, with a commitment to patient safety, teamwork and continuing professional development.

Desirable experience

Asthma, COPD and spirometry management.

Infection prevention and control leadership.

Clinical audit and risk management.

Contraception and sexual-health advice, ECG recording or ear irrigation.
