We are looking for an enthusiastic, compassionate and experienced Practice Nurse to join our friendly and supportive team on a 9-month fixed-term maternity cover contract. Based in the heart of Banstead, Surrey, Longcroft Clinic is a forward-thinking, high-performing training practice providing high-quality primary care services to over 10,600 patients.

Main duties of the job
Main responsibilities will include:
Delivering a comprehensive range of practice nursing services, including health assessments, screening, treatment, preventative care and health promotion.Providing evidence-based nursing care, including wound care, dressings, cervical cytology, immunisations and vaccinations (including seasonal influenza and COVID-19 programmes), travel vaccinations and NHS Health Checks.Participating in the management of long-term conditions such as diabetes, asthma, COPD, hypertension and cardiovascular disease, in line with national guidance and practice protocols.Undertaking chronic disease reviews, monitoring treatment plans and referring or liaising with GPs where appropriate.Assessing, triaging and managing patients presenting with minor illness or acute conditions within agreed protocols and scope of practice.Performing clinical procedures including venepuncture, ECGs, blood pressure monitoring, urine testing, specimen collection, wound management and other diagnostic tests as appropriate.Providing lifestyle advice and patient education to support self-management, disease prevention and health improvement.Identifying health risks and safeguarding concerns, escalating appropriately and working collaboratively with GPs and other healthcare professionals.Supporting infection prevention and control, medicines management, cold chain compliance and clinical governance within the practice.

Longcroft Clinic is seeking an enthusiastic, experienced and patient-focused Practice Nurse to join our friendly multidisciplinary team on a fixed-term maternity cover contract. The successful candidate will work as an autonomous practitioner within their professional scope while collaborating closely with GPs, healthcare professionals and administrative staff to deliver high-quality, evidence-based nursing care to our registered patient population.
The Practice Nurse will provide a comprehensive range of clinical services including chronic disease management, cervical screening, immunisations, wound care, health promotion, NHS Health Checks and management of minor illness, whilst supporting the practice in delivering safe, effective and responsive primary care.
Main Duties and ResponsibilitiesClinical ResponsibilitiesDeliver high-quality nursing care in accordance with the NMC Code, national guidance and practice policies.Undertake comprehensive patient assessments and provide appropriate treatment, advice and follow-up.Deliver cervical cytology in line with the NHS Cervical Screening Programme.Administer childhood, adult and travel immunisations and seasonal vaccination programmes including influenza and COVID-19.Undertake wound assessment, dressings, suture and clip removal, compression bandaging and leg ulcer management.Perform venepuncture, ECGs, blood pressure monitoring, urinalysis, spirometry (where trained) and other diagnostic procedures.Provide NHS Health Checks and cardiovascular risk assessments.Assess, triage and manage patients presenting with minor illness within agreed clinical protocols.Recognise deteriorating patients and escalate concerns appropriately.Long-Term Condition ManagementUndertake reviews for patients with long-term conditions including:DiabetesAsthmaCOPDHypertensionCardiovascular diseaseMonitor treatment plans and ensure evidence-based management.Support achievement of QOF and other quality improvement targets.Refer patients to GPs or other healthcare professionals where appropriate.Health PromotionPromote healthy lifestyles and disease prevention.Provide smoking cessation, weight management, alcohol reduction and exercise advice.Educate patients to improve self-management of long-term conditions.Support national screening and vaccination programmes.Patient CareDeliver compassionate, patient-centred care.Maintain patient dignity, privacy and confidentiality.Advocate for patients and involve them in decisions about their care.Identify safeguarding concerns and follow practice safeguarding procedures.Care CoordinationArrange investigations including blood tests, urine samples and swabs.Interpret results within competence and escalate where necessary.Refer patients to community, secondary care and allied health professionals.Liaise effectively with multidisciplinary teams to ensure continuity of care.Infection Prevention and Clinical GovernanceMaintain high standards of infection prevention and control.Ensure compliance with cold chain and vaccine storage requirements.Participate in clinical audits, significant event reviews and quality improvement projects.Report incidents, near misses and clinical risks in accordance with practice policy.Information ManagementMaintain accurate, timely and comprehensive clinical records using EMIS Web.Ensure all consultations are appropriately coded.Maintain confidentiality in accordance with GDPR and NHS Information Governance standards.Professional ResponsibilitiesMaintain NMC registration and professional revalidation requirements.Participate in appraisal, mandatory training and continuing professional development.Keep up to date with current clinical guidelines and best practice.Work within professional competence and seek support where appropriate.Contribute to the development of nursing protocols and clinical pathways.Team WorkingWork collaboratively within the multidisciplinary practice team.Support colleagues and contribute to a positive working environment.Participate in practice meetings, education sessions and service development.Supervise or support healthcare assistants and student nurses where appropriate.General ResponsibilitiesThe post holder will:

Person Specification
Experience Essential
Registered Adult Nurse (Level 1) with current NMC registration. Experience working as a Practice Nurse within a General Practice or Primary Care setting. Competent in delivering chronic disease management clinics (e.g. Diabetes, Asthma, COPD, Hypertension). Qualified and competent in cervical cytology (smear taking). Competent in administering childhood and adult immunisations and vaccinations. Experience in wound care, dressings, venepuncture and routine clinical procedures. Knowledge of infection prevention and control and clinical governance. Ability to assess, triage and manage patients within agreed clinical protocols. Competent IT skills with experience using electronic patient record systems (e.g. EMIS Web or equivalent).
Qualifications Essential
Qualifications Essential Registered Adult Nurse (Level 1) with current registration with the Nursing and Midwifery Council (NMC). Evidence of continuing professional development (CPD).
