Main duties of the job As an Advanced Clinical Practitioner (ACP) in the East Sussex Home Visiting Service, your role involves a combination of clinical expertise, autonomy, and decision-making. You will assess complex presentations, manage uncertainty, and make timely decisions that directly impact patient outcomes. This role enables you to use your full clinical skillset while assisting patients in accessing appropriate care. At IC24, you will deliver high-quality clinical assessments and treatment in patients' homes, supporting urgent and primary care services. You must be an experienced clinician, working within your professional scope, providing assessment, diagnosis, treatment, and prescribing for acute, urgent, and undifferentiated conditions. You will help reduce unnecessary hospital admissions and improve patient outcomes by ensuring patients receive the right care in the most suitable setting. The role involves working autonomously, with a focus on delivering advanced clinical care in community settings, making safe, evidence-based decisions, independent prescribing, supporting continuity of care, contributing to service improvement, and developing clinical excellence.

What you'll focus on

Delivering advanced clinical care in the community

You'll assess, diagnose and manage patients in their own homes, providing high-quality clinical care for a wide range of urgent and primary care presentations.

Making safe, evidence-based decisions

You'll use advanced clinical reasoning skills to determine appropriate treatment plans, referrals, investigations and follow-up care while managing clinical risk effectively.

Independent prescribing

As an Independent Prescriber, you'll prescribe medication following clinical assessment and examination, ensuring safe and effective patient-centred care.

Supporting continuity of care

You'll work closely with GPs, nurses, allied health professionals, NHS 111, community services and secondary care partners to ensure patients receive coordinated and seamless care.

Contributing to service improvement

You'll support audits, clinical governance activities, shared learning and quality improvement initiatives that enhance patient outcomes and service delivery.

Developing clinical excellence

You'll maintain your continuing professional development, contribute to mentoring and shared learning, and help promote best practice across the wider organisation.

This role forms part of our East Sussex Home Visiting Service, supporting patients across the community. You'll work as part of a highly skilled multidisciplinary team delivering responsive care where it is needed most, helping patients avoid unnecessary hospital attendance wherever appropriate.

To be successful in this role, you'll need:

Professional registration with the NMC, HCPC or GPhC

Independent Prescriber qualification

Advanced assessment and minor illness qualification (Level 6 or 7)

Proven experience of autonomous clinical practice

Strong clinical decision-making skills and ability to manage complex presentations

Excellent communication and interpersonal skills

Commitment to continuous professional development and reflective practice

Evidence of service improvement and innovation
