Job SummaryThis position is located within the Department of Nursing Services at Kayenta Service Unit in Kayenta, AZ. The position provides expertise in assessing, planning, implementing, and evaluating care and education for individuals with or at risk for Type 2 diabetes. It also provides staff training and clinical consultation to support coordinated, high-quality, cost-effective diabetes care and improved patient self-management. A REAL ID will be required, in accordance with 6 C.F.R. 37.5 (2021).
QualificationsTo qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. BASIC REQUIREMENT(S): Education A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education(external link), Council on Accreditation of Nurse Anesthesia Educational Programs(external link), Accreditation Commission for Midwifery Education(external link), or an accrediting body recognized by the U.S. Department of Education(external link) at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools (external link) is required for individuals who graduated from foreign nursing schools. Licensure- For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. In addition, to the Basic Requirements, you must also meet the Minimum Qualifications stated below- MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: Specialized experience include: Independently managing diabetes case management programs; assessing, planning, implementing, and evaluating care for patients with prediabetes, Type 1, Type 2, and gestational diabetes; providing comprehensive diabetes self-management education; coordinating interdisciplinary care with providers and specialty clinics; maintaining patient registries and monitoring clinical quality metrics; performing clinical procedures such as medication administration, blood glucose testing, A1C testing, and patient assessments; developing case management guidelines; participating in performance improvement and accreditation activities; creating culturally appropriate patient education materials; implementing diabetes prevention programs; and providing consultation and in-service training to multidisciplinary healthcare teams. Major DutiesTotal Compensation Package - Check out IHS's outstanding total compensation package for this job: Registered Nurse Total Compensation | Pay (ihs.gov) Assess, plan, implement, and evaluate care for patients with or at risk for diabetes, following IHS Diabetes Standards of Care and coordinating treatment plans with the healthcare team. Educate patients, families, and groups on diabetes pathophysiology, medications, nutrition, exercise, blood glucose monitoring, and prevention and management of acute and chronic complications. Collaborate with providers and specialty departments (e.g., Optometry, Dental, and other clinics) to ensure patients receive recommended screenings, follow-up care, and services to prevent diabetes-related complications. Monitor patient registries, conduct follow-up for overdue or high-risk patients, develop case management guidelines, evaluate patient outcomes, and participate in performance improvement, audits, and policy development.
