Job SummaryNurse (Case Manager) for the Colorado River Service Unit in Parker, AZ. Position will provide professional nursing case management, utilization management, and discharge planning services by independently planning and executing inpatient case management activities, coordinating care for patients with complex acute and chronic health conditions, and serving as a clinical resource to other staff. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. 1) BASIC REQUIREMENT(S): Education: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained. Degree from Foreign Nursing School: Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools. Licensure For all grade levels and positions, applicants must have passed the National Council Licensure Examination. In addition, they must possess a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. 2) In addition to Basic Requirements, you must also meet the MINIMUM QUALIFICATIONS stated below: GS-09 Grade Level: Possession of a master's degree in professional nursing; OR Your resume must demonstrate at least one full year of professional nursing experience (equivalent to the GS-7 grade level or above) and possession of a diploma, associate degree, or bachelor's degree in Nursing from a professional nursing program. Examples of experience include: applies case management essential activities; assessment, planning, implementation, coordination, monitoring, evaluation, outcomes across the continuum of care. The continuum of care matches ongoing needs of individuals being served by the case management process with the appropriate level and type of health, medical, financial, legal and psychosocial care for services from the PRC setting and/or across multiple settings. GS-11 Grade Level: Successful completion of a PhD or equivalent doctoral degree from a professional nursing educational program or related medical science field; OR Your resume must demonstrate at least one full year of professional nursing experience (equivalent to the GS-9 grade level) and possession of a diploma, associate degree, bachelor's degree, or master's degree from a professional nursing educational program. Examples of experience include: developing a tracking system for patient care coordination and care management across the continuum; independently plans and provides nursing care based on professional nursing practices and medical protocols or guidelines for patients; collaborating with the physicians to develop clinical protocols and guidelines that establish standards of care; providing case management for patients by coordinating services to meet patient's care goals; providing oversight to ensure compliance and appropriateness of care. Major DutiesInitiates the case management process upon admission and conducts a comprehensive inpatient assessment. Monitors and adjusts the discharge plan throughout the episode of care in response to changes in patient's condition. Arranges post-acute and community services among providers, agencies, the patient, family, and caregivers. Ensures that medications have been reconciled, that patient and caregiver education has been provided. Serves as a clinical resource to lower-graded nursing staff and interdisciplinary team members. Applies advanced professional registered nursing knowledge and independent clinical judgment to conduct bedside and chart-based assessments.
