Bank Pharmacist Fitzwilliam Hospital, Peterborough

Bank hours (typically Weekdays: 09:00–17:00; Saturdays: 09:00-16:00hrs)

An exciting opportunity has arisen for a highly motivated Bank Pharmacist to work at Fitzwilliam Hospital. In this role you will be an integral part of a team who are committed to delivering the highest quality clinical outcomes in a supported environment, where you will be valued for your skills and expertise.

Duties will include:

• Undertake medicines reconciliation, medication history taking, and prescription review.

• Identify and manage pharmaceutical care issues to support the safe and effective use of medicines.

• Liaise with patients, carers, doctors, nurses, and other healthcare professionals as appropriate.

• Provide medicines advice, including dosage recommendations where required.

• Monitor and document adverse drug reactions (ADRs) and ensure appropriate communication with patients and the healthcare team.

• Discharge Planning

• Support safe and timely patient discharge by reviewing medication requirements, ensuring prescription accuracy, providing patient counselling, and managing patients' own medicines as appropriate.

What you'll bring with you:

• Degree in Pharmacy

• Registered with the General Pharmaceutical Council (GPhC)

• Effective written and verbal communication skills

• Ability to work both independently and as part of a multidisciplinary team

• Flexible and adaptable approach to work

• Good customer service and computer skills

• Previous hospital experience (desirable)
