*Key Responsibilities*:
* Provision of *pharmaceutical care*. * Perform *prescription verification* (technical and therapeutic) * Conduct *medication reviews* and complete required documentation * Identify and resolve *drug therapy problems* and document pharmaceutical opinions * Provide *immunizations and injections* (where applicable) * Collaborate with the pharmacy team to deliver *optimal patient care* * Support and contribute to *innovative pharmacy services and programs* * Practice to the *full scope of practice* in accordance with the Ontario College of Pharmacists

*Qualifications/Experience:*
* Licensed *Part A Pharmacist in good standing* with the Ontario College of Pharmacists (required) * Authorization to practice pharmacy in Canada (required) * Community pharmacy experience (preferred, but *not required*) * *New graduates and interns are encouraged to apply* * Injection certification and CPR/First Aid (required)
