We are looking for a motivated and compassionate Band 6 Children & Young People (CYP) Continuing Care Nurse to join our Continuing Care Team. This is an exciting opportunity to play a key role in ensuring children and young people with complex health needs receive high-quality, person-centred care that enables them to achieve the best possible outcomes. Working within a supportive multidisciplinary team, you will be responsible for assessing, coordinating, reviewing, and case managing children and young people who are eligible for Continuing Care, ensuring care is delivered in line with the National Framework for Children and Young People's Continuing Care.

About the Role
As a CYP Continuing Care Coordinator, you will manage a caseload of children and young people with complex health needs, undertaking comprehensive assessments, coordinating care packages, and reviewing ongoing care to ensure services remain appropriate and effective. Working closely with the CYP Continuing Care Lead and a wide range of health, social care, education, and voluntary sector partners, you will help deliver safe, effective, and high-quality services while supporting the ongoing development of the Continuing Care programme. You will also contribute to service improvement initiatives, providing high-quality information, analysis, and reporting to support informed decision-making and effective resource planning.
Key Responsibilities
Assess eligibility for Children and Young People's Continuing Care in line with national guidance.
Coordinate, review, and case manage Continuing Care packages for children and young people with complex health needs.
Develop and maintain effective relationships with children, families, carers, and partner organisations. Work collaboratively with multidisciplinary teams to ensure coordinated, person-centred care.
Support the planning, implementation, monitoring, and evaluation of Continuing Care programmes and projects.
Prepare reports, correspondence, and documentation to support service delivery and governance.
Analyse complex information to support decision-making and service development.

Person Specification
Knowledge Essential
A competent CYP expert with excellent knowledge and experience working with complex needs and terminal illness. Experience undertaking complex assessment and evaluation of care needs to determine appropriate support packages that are personalized to need and outcomes identified. Ability to problem solve complex issues in a constructive and professional manner within team and other agencies. Demonstrates awareness of the importance of working as part of a multi-disciplinary team. Communicates effectively verbally, in writing and in electronic formats. Communicates with CYP and family/carers in a caring and empathetic manner. A high level of understanding and experience of CYP with complex needs, terminal illness and life changing situations in acute/community settings. An in-depth awareness of NHS Continuing Care and National Frameworks and NHS 5 Year Plan.
Desirable
Experience of Band 6 role. Experience with working in the community.
Qualifications Essential
Educated to degree level in relevant subject or equivalent level of experience of working at a similar level in specialist area. LDN/RSCN/RN (Child). Accredited teaching course.

Problem solving skills and ability to respond to sudden unexpected demands. Strategic thinking ability to anticipate. Attention to detail combined with the ability to extract key messages from complex analysis. Independent thinker with demonstrated good judgement, problem-solving and analytical skills. Takes decisions on difficult and contentious issues where they may be a number of courses of action.
Autonomy Essential
Ability to work on own initiative and organise own workload without supervision working to tight and often changing timescales. Can work flexibly in a changing ICB environment. Ability to work at various locations and occasionally out of county, work remotely from laptop as needed and adapt working situation to undertake assessments/reviews.
Communication Essential
Clear communicator with excellent writing, report writing and presentation skills; capable of constructing and delivering clear ideas and concepts concisely and accurately for diverse audiences. Evidence of success in efficient and effective project and programme management. Skills for communication on complex matters and difficult situations, requiring persuasion and influence. Skills for nurturing key relationships and maintaining networks with all multiagency teams.
