Please note: at this stage, the role is open only to colleagues currently employed within Kent and Medway NHS Trusts, the Kent and Medway Integrated Care Board (ICB) and Medway Community Healthcare. Nursing vacancies are also open to Student Nurses and Newly Qualified Nurses.
This is an exciting opportunity to join the Kent Health Visiting Service as a Family Partnership Practice Lead(FPPL) to work within the Swale District. The FPPL has a key role in embedding the Family Partnership Modelwithin Kent Health Visiting Services under the guidance of the Family Partnership Programme Manager. TheFamily Partnership Model is an innovative approach which is evidence based and enables parents and familiesto overcome their difficulties, build strengths and resilience and fulfil their goals more effectively.Working as part of the local Health Visiting team, the Family Partnership Practice Lead will facilitate the earlyidentification of families in need of additional support, provide enhanced support while delivering the FamilyPartnership Programme. The leads will support the delivery of the Healthy Child Programme, manage a smallcaseload of families, supporting them ante-natally and until their infant is a year old.
Main duties of the job
The post holder will also have a lead role in embedding the Family Partnership Model within district healthvisiting practice as well as providing clinical supervision, fulfil both leadership, and some line managementresponsibilities to the team. You will contribute to a safe and effective operational running of the HealthVisiting Service.The work with clients is intensive and challenging but provides a real opportunity to make a difference to theirlives. You need to be a resilient and flexible Health Visitor, with excellent communication, team leadershipskills, while acting as a role model to uphold high professional standards and promote high-quality,person-centred care.

Person Specification
Qualification Essential
Level 6 Degree in Nursing or Midwifery as well as a Specialist Community Public Health Nurse with a Health Visiting Qualification Registered Nurse or Midwife with the Nursing and Midwifery Council, with a current registration.
Experience Essential
Ability to work effectively within your area of practice, supported by relevant experience. As well as managing complex caseloads and supporting/ mentoring others.
Knowledge and skills Essential
Have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence-based procedures and clinical effectiveness frameworks Know when you are within your own limits and when to escalate
Abilities Essential
Be adaptable in work as things change as part of working in a wider team.
Desirable
Have an awareness of developments within the NHS and National Service Framework
Working Environment Essential
Have a full, valid UK driving licence issued by an EU country and the ability to travel independently
