About this role

Job summary

King's Lynn PCN is seeking two experienced first contact physiotherapists (FCPs) to deliver a high-quality musculoskeletal (MSK) assessment and management service for patients across our primary care network (PCN), which serves approximately 70,000 patients.

Based primarily at King's Lynn Health Hub, with sessions across participating GP practices, you will provide expert face-to-face assessment, diagnosis, treatment and rehabilitation for patients presenting with MSK and other appropriate conditions. Working autonomously within your professional scope of practice, you will support patients to self-manage their conditions, reduce unnecessary referrals and improve access to specialist MSK care.

The post holder will utilise SystmOne across the PCN, manage their own clinical caseload, complete referrals and investigations where appropriate, and contribute to the ongoing development of high-quality, patient-centred services

Main duties of the job

Clinical Responsibilities

Assess, diagnose, triage and manage patients presenting with musculoskeletal and other appropriate conditions using evidence-based practice. Undertake comprehensive patient assessments and formulate individualised treatment and rehabilitation plans. Deliver predominantly face-to-face consultations. Accept referrals from GP clinicians and other healthcare professionals, together with appropriate self-referrals via practice reception. Provide first-line treatment, education and exercise programmes to promote self-management. Design rehabilitation programmes that improve mobility, independence and physical activity while reducing unnecessary pharmacological intervention. Request and interpret appropriate investigations (including X-rays) within scope of practice, recognising their limitations and acting appropriately on results. Make referrals to secondary care, community MSK services, rehabilitation services and social prescribing where appropriate. Maintain accurate, timely clinical records using SystmOne. Escalate clinical concerns appropriately and work within professional competence at all times.Please see the attached job description for further information on the main duties and responsibilities of this role.

About us

Based in the new purpose-built Kings Lynn Health Hub, the Kings Lynn Primary Care Network serves a population of approximately 71,000 patients residing across the Kings Lynn area within west Norfolk.

Working collaboratively with other partners to strengthen and support the delivery of primary care services, the primary care network (PCN) consists of 3 core GP practices Vida Healthcare, St James Medical Practice, Southgates Medical and Surgical Centre & the Woottons Surgery.

The PCN is led by a clinical director, Dr Caroline Delves.

Person Specification

To be considered for the position, you will require the following: Essential

BSc (Hons) in Physiotherapy or equivalent recognised qualification Master's degree or equivalent specialist knowledge, skills and experience Demonstrable Level 7 capability in MSK assessment, diagnosis and treatment HEE-recognised First Contact Practitioner (FCP) status Current HCPC registration Eligibility for Chartered Society of Physiotherapy (CSP) membership Significant experience managing musculoskeletal caseloads independently Ability to prioritise and manage a diverse clinical workload Experience using SystmOne or similar electronic patient record systems Excellent communication, interpersonal and organisational skills Right to work in the UK. Desirable

Previous experience working in primary care Experience delivering remote consultations (telephone consultations) Evidence of service development, audit or quality improvement work Experience working across multiple clinical sites Leadership, supervision or mentoring experience Independent prescribing qualification, or willingness to work towards one
