Main duties of the job
As a Practice Nurse, you will manage your own nurse-led clinics while working as part of a supportive multidisciplinary team, delivering high-quality, patient-centred care across a broad range of clinical services.
Your responsibilities will include:
Delivering chronic disease management for patients with long-term conditions, including diabetes, asthma and COPD.Providing cervical screening in line with national guidelines.Administering routine and seasonal immunisations and vaccinations.Delivering travel health advice and vaccinations.Providing women's and men's health services, including health promotion and preventative care.Undertaking wound care, dressings, and other treatment room procedures.Educating and supporting patients to improve their health, wellbeing.Maintaining accurate clinical records and contributing to the delivery of safe, effective, evidence-based care.Working closely with experienced GPs, nurses, healthcare assistants, pharmacists, and administrative colleagues, you will be part of a collaborative and respectful team where everyone's contribution is recognised and valued. Job description Job responsibilities
As a Practice Nurse, you will provide high-quality, evidence-based nursing care while working collaboratively with the multidisciplinary team to deliver safe, effective, and patient-centred services.
Clinical ResponsibilitiesManage patients with long-term conditions by running nurse-led chronic disease clinics and supporting effective call and recall systems.Deliver national screening programmes, including cervical cytology, in accordance with current guidelines.Administer routine, seasonal, and travel immunisations and vaccinations, providing travel health advice where appropriate.Assess, treat, and manage patients presenting with minor injuries and minor illnesses within your scope of practice.Provide contraceptive services and support GPs in delivering sexual and reproductive healthcare.Undertake wound care, dressings, and other treatment room procedures.Perform, interpret, and act upon the results of agreed clinical investigations and diagnostic tests.Promote healthy lifestyles and deliver health promotion and disease prevention initiatives to support patients in managing their health and wellbeing.Communicate sensitively and compassionately with patients, relatives, and carers, including providing appropriate support when discussing difficult or distressing information.Act as a chaperone in accordance with practice policy when required.Administration and QualityMaintain accurate, timely, and confidential patient records using the clinical system, ensuring appropriate clinical coding in line with practice procedures.Complete all clinical documentation accurately and in accordance with professional, legal, and organisational requirements.Contribute to clinical audit, quality improvement initiatives, and research projects to support continuous service development.Assist in the development and implementation of practice policies, procedures, and service improvement plans.Professional ResponsibilitiesMaintain current registration with the Nursing and Midwifery Council (NMC) and practise in accordance with the NMC Code.Maintain and develop professional knowledge and skills through continuing professional development (CPD), ensuring your Personal Development Plan (PDP) remains up to date.Work within current clinical guidelines, infection prevention and control standards, safeguarding procedures, and all relevant legislation, including Health and Safety and COSHH regulations.Promote and maintain a safe, supportive, and therapeutic environment for patients, colleagues, carers, and partner organisations.Demonstrate professionalism, compassion, and respect in all interactions with patients, colleagues, and external agencies.Attend and actively participate in practice meetings, mandatory training, and ongoing clinical education.Undertake any other reasonable duties appropriate to the role, commensurate with your skills, experience, and professional competence.
Person Specification
Knowledge and Skills Essential
Sound knowledge of current primary care nursing practice and evidence-based guidelines. Understanding of infection prevention and control, safeguarding, confidentiality, and information governance. Commitment to delivering high-quality, patient-centred care. Ability to promote health, support self-management, and educate patients in managing long-term conditions.
Desirable

Experience Essential
Experience in managing patients with long-term conditions. Experience of delivering cervical screening services. Experience of providing health promotion and preventative care. Excellent communication and interpersonal skills, with the ability to build effective relationships with patients and colleagues. Ability to work independently while contributing effectively as part of a multidisciplinary team. Good organisational skills, with the ability to prioritise workload and manage nurse-led clinics. Competent in maintaining accurate clinical records and using electronic patient record systems.
Desirable
Previous experience working within a GP practice or primary care setting. Experience of family planning and contraceptive services. Experience in delivering smoking cessation advice and support. Evidence of recent continuing professional development relevant to primary care. Experience of using EMIS Web clinical system.
Qualifications Essential
Essential Registered General Nurse (RGN) with a current Nursing and Midwifery Council (NMC) registration. Accredited training in the management of long-term conditions, including chronic disease management. Recognised qualification in cervical cytology (smear taking).
Desirable
Desirable Up-to-date training in childhood immunisations and vaccinations. Additional qualifications or training relevant to primary care nursing.
