E4 Primary Care Network (PCN) delivers a well-established and highly regarded Enhanced Access and Same Day Access Service on behalf of its member GP practices across Chingford. The service provides timely, high-quality care to registered patients through a collaborative, multidisciplinary team approach, helping to improve access to primary care services and patient outcomes.
Operating from a dedicated hub in the centre of the E4 PCN, the service offers a wide range of clinical appointments and specialist services, including General Practice, Same Day Access, First Contact Physiotherapy, NHS Health Checks and Blood Pressure Checks, Wound Care, Phlebotomy, Sexual Health Services, Spirometry, Nursing Services, Minor Surgery, and Joint Injection Clinics.
The service is supported by an experienced multidisciplinary team comprising GPs, Advanced Clinical Practitioners, Nurses, Healthcare Assistants, Physiotherapists, GP Assistants, Administrators, and other allied healthcare professionals. Working closely with our member practices, the team delivers integrated, patient-centred care while supporting the wider objectives of the Primary Care Network.
We are seeking reliable motivated and patient-focused GP's with good time keeping skills to join our team and contribute to the continued development and delivery of accessible, high-quality primary care services for the residents of Chingford.

The GP will provide high-quality clinical care within the E4 PCN Enhanced Access and Same Day Access Service, delivering safe, effective and patient-centred consultations for individuals referred via GP practices, NHS 111 and urgent care services. The role involves the assessment, diagnosis and management of a wide range of acute and primary care presentations, ensuring care is delivered in line with national and local evidence-based guidelines.
The postholder will be responsible for making appropriate prescribing decisions, requesting and interpreting investigations, and initiating timely referrals to secondary care or other healthcare services where clinically indicated. Accurate and comprehensive clinical record keeping is essential for all patient interactions.
Working as part of a multidisciplinary team, the GP will collaborate closely with colleagues across primary care and allied health services to ensure coordinated and high-quality patient care. The role also requires clear communication with patients regarding their care and the scope of the service, as well as maintaining professional standards in line with GMC requirements, local policies and clinical governance frameworks.

E4 Primary Care Network (PCN) is a collaborative network of GP practices serving the Chingford area in North East London. The PCN is responsible for a registered patient population of over 51,000 people, with a growing list size of approximately 5,400 patients, reflecting the continued expansion and development of local primary care services.
The network is made up of five established member practices, working closely together to deliver coordinated, high-quality care to the local community: Chingford Medical Centre, Churchill Medical Practice, The Old Church Surgery, The Ridgeway Surgery, and Handsworth Medical Practice. These practices maintain strong, collaborative working relationships, enabling the delivery of integrated and patient-focused services across the network.
The population served by the PCN is diverse, spanning all age groups from young families to older adults with complex health needs. The network also provides care and support for over 350 residents living in local care homes, ensuring continuity of care and proactive management for some of the most vulnerable members of the community.

The General Practitioner will provide high-quality, patient-centred care as part of the Enhanced Access and Same Day Access Service, working collaboratively within a multidisciplinary team to deliver timely and effective primary care services to patients registered with E4 PCN member practices.
Key responsibilities include:
Undertake face-to-face, telephone and remote consultations for patients booked through member GP practices, NHS 111 and Urgent Care Centre (UCC) pathways.
Assess, diagnose and manage a broad range of acute and routine primary care presentations, providing safe, effective and evidence-based treatment.
Provide comprehensive and competent assessment, clinical advice and treatment for primary care conditions, following national and local clinical guidelines and best practice standards.
Maintain accurate, timely and comprehensive clinical records of all patient consultations using the relevant clinical systems.
Request, interpret and act upon investigations as appropriate, including pathology and diagnostic tests.
Prescribe medications safely and appropriately in accordance with national and local prescribing guidelines, formularies and policies.
Initiate and manage referrals to secondary care, community services, diagnostic services and other healthcare agencies as clinically appropriate.
Ensure appropriate follow-up arrangements are made and communicate relevant information to patients and their registered GP practice.
Advise patients regarding the scope and limitations of the Enhanced Access and Same Day Access Service and signpost or direct them to alternative services where appropriate.
Manage clinical risk effectively, recognising and responding appropriately to urgent, emergency and safeguarding concerns.
Work collaboratively with GPs, Advanced Clinical Practitioners, Nurses, First Contact Physiotherapists, Pharmacists and other members of the multidisciplinary team to deliver integrated patient care.
Provide clinical support and advice to colleagues where appropriate and contribute to effective multidisciplinary team working.
Participate in clinical governance activities, including significant event reviews, audits, quality improvement initiatives and service development.
Maintain professional registration, appraisal, revalidation and mandatory training requirements.
Comply with all organisational policies, information governance requirements, safeguarding procedures and infection prevention and control standards.
Maintain appropriate professional standards in accordance with the General Medical Council (GMC) Good Medical Practice guidance and any locally agreed clinical standards and protocols.
Contribute to the ongoing development and improvement of the Enhanced Access and Same Day Access Service to ensure high standards of patient care and service delivery.
The postholder will undertake any other duties commensurate with the role and grade as reasonably required to support the effective operation of the service.
