Job summary
We are seeking a highly skilled and autonomous Advanced Nurse Practitioner (ANP) to join our Team. This role is pivotal in providing high-quality, patient-centred care in a fast-paced, supportive general practice environment.
As an ANP, you will lead on delivering undifferentiated primary care to a varied patient population, working independently but with access to clinical support from the practice team. The successful candidate will demonstrate excellence in clinical assessment, decision-making, and prescribing, with a commitment to upholding the standards of care and safety expected by Gateshead Healthcare.

Key Responsibilities
The Advanced Nurse Practitioner (ANP) will utilise advanced clinical knowledge and skills to independently assess, diagnose, treat, prescribe, and provide expert advice to patients through face-to-face consultations, delivering evidence-based care that is safe, effective, and patient-centred.
The role includes responsibility for:
Delivering undifferentiated patient care during morning and afternoon clinical sessions.
Providing timely follow-up of patients and responding appropriately to ongoing case management requirements.
Practising autonomously within professional competence, adhering to current NICE guidance, national standards, and best practice.
Managing complex patient cases, ensuring appropriate clinical pathways are followed and tailored to meet the needs of both patients and the service.
Independently prescribing medications within scope of practice, ensuring safe, evidence-based prescribing in accordance with current legislation and professional standards.

Clinical Responsibilities
Patient Care
Deliver advanced and complex clinical care, providing assessment, diagnosis, treatment, and prescribing for patients with acute, chronic, and complex conditions.
Autonomous Practice
Work independently within the scope of practice to manage complete clinical episodes, using advanced diagnostic skills and clinical judgment.
Case Management
Manage patient cases from initial assessment to conclusion, including follow-ups and referrals, ensuring continuity of care.
Prescribing
Utilise prescriptive authority responsibly following current guidelines and protocols to enhance therapeutic outcomes for patients.
Home visiting
Undertake home visiting as and when required, including triage of home visits.
Leadership and Guidance
Mentorship
Provide mentorship and supervision to nursing staff, trainee advanced practitioners, and other healthcare professionals to enhance team competencies.
Professional Development
Engage in continuous professional development to keep abreast of medical advancements and to further develop clinical skills.
Research and Quality Improvement
Implement quality improvement initiatives and ensure compliance with health care standards and best practices in patient care.
Educational Activities
CPD Activities Facilitate and participate in Continuous Professional Development (CPD) activities within the practice.
Administrative and Organisational Duties
Record Keeping
Maintain comprehensive and accurate patient records using EMIS Web.

Person Specification
Experience Essential
Extensive experience in managing complete episodes of clinical care including assessment, diagnosis, treatments, and prescribing Advanced clinical skills in a primary care setting, with proficiency in using clinical diagnostic tools and conducting complex clinical procedures
Qualifications Essential
Masters degree in advanced Practice, or equivalent post-graduate qualification, or full accreditation or eportfolio Registration with the Nursing and Midwifery Council (NMC) or Health and Care Professions Council (HCPC) or other regulatory body as appropriate
Desirable
Further qualifications in a specialist area such as diabetes. Experience and competency in chronic disease management.
