This role is for 3 days working at Market Rasen Surgery and 1 day working at Woodhall Spa New Surgery.
The Specialist Practice Nurse is responsible for providing high-quality, evidence-based nursing care to patients with long-term conditions, including diabetes, asthma, and chronic obstructive pulmonary disease (COPD). The post holder will also deliver comprehensive travel health services, including risk assessments, travel advice, and vaccinations. They will work collaboratively with the multidisciplinary team to promote health, prevent disease, and improve patient outcomes.

5. Main duties (working to strict protocols and standard operating procedures)
Diabetes Management
Conduct annual and interim diabetes reviews in accordance with national and local guidelines. Assess glycaemic control using HbA1c results and provide appropriate education and support. Monitor blood pressure, weight, BMI, foot health, and cardiovascular risk. Perform diabetic foot assessments and refer patients when required. Review medications within scope of practice and support treatment optimisation. Educate patients on blood glucose monitoring, diet, exercise, and lifestyle modifications. Develop personalised care plans to support self-management. Identify and refer patients requiring specialist diabetes services.
Respiratory Care (COPD & Asthma)
Conduct comprehensive asthma and COPD reviews. Perform and interpret spirometry (where appropriately trained and accredited). Assess inhaler technique and optimise inhaler therapy. Develop and update personalised asthma action plans and COPD self-management plans. Identify patients at risk of exacerbations and implement appropriate management. Provide smoking cessation advice and referrals. Support vaccination programmes, including influenza and pneumococcal vaccinations. Refer patients requiring specialist respiratory assessment when appropriate.
Travel Health
Undertake comprehensive travel risk assessments. Provide destination-specific travel health advice. Administer travel vaccinations in accordance with national guidance and Patient Group Directions (PGDs) or prescribing qualifications. Advise on malaria prevention, food and water hygiene, insect bite prevention, and other travel-related health risks. Maintain accurate vaccination records and documentation. Ensure vaccines are stored and managed according to cold chain requirements.
Person Specification
Qualifications Essential
Registered Nurse (Adult) with current NMC registration. Recognised qualification or accredited training in diabetes management. Accredited respiratory disease management training. Evidence of ongoing CPD.
Desirable
Travel health training. Nurse Prescriber
