We're looking for an enthusiastic and compassionate Practice Nurse to join our well-established rural dispensing GP practice for 16 hours per week (Tuesday and Friday). As a Practice Nurse, you'll deliver a broad range of treatment room services while supporting patients with Long-Term Conditions (LTCs) through structured reviews, health promotion and preventative care. You'll work alongside our GPs, Nursing Associate and wider practice team to provide high-quality, evidence-based care and contribute to the achievement of Quality and Outcomes Framework (QOF) and Enhanced Service targets.

Main duties of the job
As our Practice Nurse, you'll provide a high standard of clinical care to patients across a wide range of treatment room and long-term condition services.
Your main duties will include:
Delivering nurse-led clinics for patients with Long-Term Conditions (LTCs), including annual reviews and supporting QOF delivery.
Providing treatment room services such as wound care, dressings, venepuncture, ECGs, blood pressure monitoring, injections and specimen collection.
Administering childhood, adult and seasonal vaccinations and immunisations, and undertaking cervical screening (where qualified).
Promoting healthy lifestyles through patient education, NHS Health Checks and preventative care.
Maintaining accurate clinical records using SystmOne and ensuring appropriate clinical coding.
Working closely with GPs and the wider multidisciplinary team to provide safe, effective and compassionate patient care.
Supporting infection prevention and control, maintaining clinical equipment and treatment room stock, and contributing to clinical audit and service improvement.

Our Team Hibaldstow Medical Practice is a small, friendly rural dispensing practice serving approximately 3,500 patients across a wide geographical area of around 120 square miles. Our close-knit team includes two GPs, two practice nurses, a nurse associate, a healthcare assistant, eight dispensers, four receptionists, two administrators and a practice manager. We work collaboratively to provide high-quality, patient-focused care to our local community.

As a Practice Nurse at Hibaldstow Medical Practice, you will play a key role in delivering high-quality, patient-centred nursing care within our friendly rural dispensing GP practice. Working as part of a supportive multidisciplinary team, you will provide a wide range of treatment room services while helping to deliver excellent outcomes for patients with Long-Term Conditions (LTCs).
Key Responsibilities
Deliver safe, effective and evidence-based nursing care in accordance with NMC standards, practice policies and national guidance.
Assess, plan, implement and evaluate patient care within your scope of practice.
Deliver nurse-led Long-Term Condition (LTC) clinics, including diabetes, asthma, COPD, hypertension and cardiovascular disease (depending on competencies).
Undertake structured annual reviews and support the achievement of Quality and Outcomes Framework (QOF) and Enhanced Service targets.
Carry out NHS Health Checks and provide health promotion and lifestyle advice.
Undertake treatment room duties including:
Wound care and dressings
Venepuncture
ECGs
Blood pressure monitoring
Injections and medication administration within competency
Specimen collection
Removal of sutures and clips
Deliver childhood, adult and seasonal immunisations and vaccinations, including influenza and COVID-19 programmes.
Undertake cervical screening (where appropriately trained).
Promote self-management and preventative healthcare through patient education.
Recognise deteriorating patients and refer or escalate appropriately.
Maintain accurate clinical records using SystmOne, ensuring appropriate coding and documentation.
Ensure treatment rooms remain clean, safe, well-stocked and compliant with infection prevention and control standards.
Participate in clinical audit, quality improvement and service development.
Work collaboratively with GPs, Healthcare Assistant and the wider multidisciplinary team to deliver outstanding patient care.
Person Specification
Qualifications Essential
Registered Adult Nurse with a current NMC registration. Evidence of continuing professional development (CPD).
Desirable
Independent Prescriber qualification. Cervical Cytology qualification. Spirometry competency. Travel Health qualification.
Experience Essential
Previous experience working within General Practice. Experience delivering high-quality nursing care within a clinical setting. Experience undertaking patient assessment and delivering evidence-based care. Experience of treatment room nursing, including wound care, venepuncture and specimen collection. Experience maintaining accurate electronic clinical records. Experience working independently and as part of a multidisciplinary team.
Desirable
Experience managing Long-Term Conditions (LTCs), including diabetes, asthma, COPD and hypertension. Experience supporting Quality and Outcomes Framework (QOF) delivery. Experience delivering NHS Health Checks. Experience administering childhood, adult and seasonal immunisations and vaccinations. Experience using SystmOne. Experience participating in clinical audit and quality improvement. Experience supervising Healthcare Assistants, Nursing Associates or student nurses.
