About this role

Job summary

We are looking to recruit an experienced clinical colleague with a passion for delivering exemplary care, to join us in looking after people affected by life-limiting illness in both the hospice and community settings.

You will be working mainly in our Palliative Advice Centre, as well as being an integral part of the medical team on the in-patient unit. Our services are based in our purpose-built hospice situated one mile from Macclesfield town centre. There is ample free parking available on site.

Role Type: Up to 37.5 hours per week, shifts across 7 days between 8am and 8pm, with the possibility of occasional rotation onto nights

Employment type: Permanent

Salary: Full Time Equivalent:

Band 7: £49,387 - £56,515

plus 30% Saturday, 60% Sunday enhancement

Main duties of the job

Our new team member will-

Deliver expert advice to healthcare professionals, patients and relatives both in person and over the phone

Undertake face to face reviews of palliative patients, working across clinical settings, inclusive of hospice & community workCoordinate care for people with a life-limiting illnessBe confident liaising with other healthcare professionals, making recommendations for changes to medication regimes/plans of care and advocating for patients and their familiesDeliver evidence based, quality care

Duties

Working mainly in our Palliative Advice Centre, as well as being an integral part ofthe medical team on the in-patient unit.Undertaking holistic assessments of patients using examination and diagnostic skillsand where appropriate, prescribing.Your excellent communication skills will be utilised to support families and carers,liaise with other healthcare professionals, and mutually agree upon plans of care.You will provide specialist advice and support to patients and carers, both face toface and via telephone. With the support of the multidisciplinary team, you will bepivotal in the co-ordination of care for palliative patients across East Cheshire, andlink in closely with specialist palliative care colleagues.And to undertake other duties as may be required and which are commensuratewith the post and grade of the role.

Other requirements Essential

Commitment to professional development Flexible approach to meet the needs of the service Ability to travel freely/ daily across the East Cheshire Hospice catchment area.

Experience Essential

A minimum of 2 years palliative care experience working at Band 6 or above

Desirable

Experience working as a Band 7 in a palliative care setting

Registered nursing qualification Adult nursing degree
