Job summary
Calderdale and Huddersfield NHS Foundation Trust are currently looking for an experienced and enthusiastic part-time Advanced Clinical Practitioner (ACP) to join our Cardiology team.
You will be the second ACP within the service and will deliver independent rapid access chest pain clinics as well as other clinical and non-clinical duties:
Teaching/Training opportunities availableACP forums and dedicated supervision to support the delivery of the Trust wide ACP strategyACP celebration events and opportunities to attend conferencesAllocated Medical Supervision
The Advanced Clinical Practitioner will be a highly experienced, knowledgeable and educated member of the team who is able to work autonomously making high level decisions to treat health needs. This is a great opportunity to join a fantastic, proactive team with space for individuals to influence service development and support the future of the SDEC service at the Trust.

Main duties of the job
Assess individuals, families and carers holistically. Communicate effectively with patients' carers and the multidisciplinary team. Use their expert knowledge and clinical judgement to decide whether to refer patients for investigations and make diagnoses. Decide on and carry out treatment, which may include the prescribing of medicines, or referring patients to an appropriate specialist. Ensure the provision of continuity of care. Assess and evaluate, with patients and /or carers, the effectiveness of the treatment and care provided and make changes as needed. Work autonomously within a wider health care team and as a lonepractitioner.

To work closely with the Cardiology medical and nursing teams to provide effective care to our Cardiology patients in both outpatient and inpatient setting.
Practice as an autonomous professional in a self-directed manner, with confidence, authority and expertise, undertaking comprehensive range of clinical procedures.
Have direct responsibility for patient caseload, ensuring all patients have an accurate plan of care, which reflects the assessment undertaken and incorporate the issues and recommendations made.
Work clinically to optimise health and wellbeing, through comprehensive history taking and physical assessment of patients.Drawing on a diverse range of knowledge and skill, determine appropriate therapeutic interventionsPlan, prioritise and manage complete episodes of care, for patients presenting with undifferentiated and undiagnosed conditions.

Registered general nurse or Allied health care professional Teaching/Assessment , Mentoring qualification Previous experience of formal/informal teaching of patients and staff

Masters in Advanced Practice or equivalent Non Medical Prescriber
