Job summary
To project manage the successful delivery of service improvement, transformation and operational change projects across Community & Mental Health services.The post holder will work with multi-disciplinary teams, pathway leads, clinicians, operational managers and external partners to ensure projects are planned, delivered and embedded effectively.The focus of the role is to improve patient pathways, capacity, demand management, service quality and overall patient experience within Community & Mental Health settings.
Main duties of the job
Support planning, implementation and evaluation of service improvement projects across Community & Mental Health pathways.
Facilitate pathway redesign work, enabling services to improve flow, access, waiting times and patient experience.
Create clear and robust project plans, timelines and delivery schedules.
Develop and deliver stakeholder communication plans to ensure transparency and engagement throughout project lifecycles.
Lead workshops, MDT sessions and facilitation events to support operational change.
Develop documentation including project scopes, operational procedures, KPIs and improvement reports.
Coordinate key project resources, escalating barriers to delivery when required.
Identify, track and manage project risks and issues.
Produce highlight reports for senior managers and service leaders.
Assist teams in implementing new operational processes, policies or models of care.
Review and map current patient pathways, working with clinicians to identify pressures and improvement opportunities.
Support the embedding of new initiatives into everyday practice through coaching, training and change support.

Person Specification
Education and Training Essential
Educated to degree level or equivalent experience in project delivery or operational service management. Evidence of ongoing professional development.
Desirable
PRINCE2 or equivalent qualification
Experience Essential
Experience of project change and delivery Experience developing documentation such as project plans, risk logs, KPIs, SOPs or improvement reports. Experience leading service improvement or operational change projects.
Desirable
NHS community or mental health experience. Experience of pathway redesign, access improvement or performance management.
Knowledge Essential
Knowledge of project management and change management principles. Understanding of Community & Mental Health pathways, operational pressures and MDT working.
Desirable
Knowledge of safeguarding, crisis response pathways or local authority interfaces.
Skills Essential
Excellent communication and stakeholder-management skills. Strong ability to analyse service data and identify improvement opportunities. Ability to manage multiple tasks and respond quickly to new issues.
Desirable
Confidence facilitating workshops and MDT discussions.
Personal Attributes Essential
Highly motivated and able to work independently Flexible and adaptable to service needs.
