Job summary
We are seeking an experienced, compassionate and motivated Practice Nurse to join our friendly and supportive GP practice. Working as part of a multidisciplinary team, you will deliver high-quality, evidence-based nursing care to our diverse patient population, supporting the management of long-term conditions, preventative healthcare, immunisation programmes and health promotion initiatives.

Main duties of the job
Deliver high-quality, evidence-based nursing care to patients within a primary care setting.Undertake chronic disease management clinics, including monitoring and supporting patients with long-term conditions.Administer immunisations, vaccinations and other national screening and prevention programmes.Provide cervical screening, wound care, dressings and other treatment room services within competency.Promote health and wellbeing through patient education, lifestyle advice and preventative healthcare.Assess, plan, implement and evaluate individualised patient care using clinical judgement.Maintain accurate and timely clinical records using the practice's clinical system.Work collaboratively with GPs and the wider multidisciplinary team to ensure safe and effective patient care.Contribute to infection prevention and control, clinical governance and quality improvement activities.Maintain professional registration, undertake continuing professional development and work in accordance with the NMC Code and practice policies.Support the delivery of enhanced services and national health initiatives as required by the practice.Ensure patient confidentiality is maintained at all times and deliver care in a compassionate, professional and respectful manner.

Key Duties and Responsibilities
Clinical Nursing Responsibilities
The Practice Nurse will:
Deliver high-quality, patient-centred nursing care to patients registered with the practice.
Assess, plan, implement and evaluate individual patient care using evidence-based practice.
Work within agreed clinical protocols, national guidance and practice policies.
Maintain professional accountability for all aspects of nursing care delivered.
Recognise deterioration in patients and escalate concerns appropriately to the GP or emergency services.
Maintain accurate, contemporaneous and legally compliant clinical records using EMIS Web.
Chronic Disease Management
The post holder will independently manage and review patients with long-term conditions in accordance with national guidance, practice protocols and Quality and Outcomes Framework (QOF) requirements.
This includes, but is not limited to:
Diabetes reviews.
Respiratory disease management (Asthma and COPD).
Hypertension monitoring.
Cardiovascular disease monitoring.
Chronic disease annual reviews.
Lifestyle interventions.
Patient education and self-management support.
Referral to other healthcare professionals where appropriate.
Identification of patients requiring GP review.
The Practice Nurse will ensure all consultations are appropriately coded to support QOF achievement and Enhanced Service requirements.
Treatment Room Services
Venepuncture/phlebotomy.
Wound assessment and management.
Dressing changes.
Leg ulcer care.
Removal of sutures and clips.
ECG recordings.
Blood pressure monitoring.
Urinalysis.
Peak flow monitoring.
Spirometry (where trained and accredited).
Height, weight and BMI assessment.
NHS Health Checks.
New patient health checks.
Lifestyle screening.
Womens Health
Undertake cervical cytology in accordance with the National Cervical Screening Programme.
Provide contraception advice within scope of practice.
Support womens health clinics.
Undertake menopause and HRT monitoring where appropriate and within competency.
Provide breast awareness education.
Signpost patients appropriately for specialist services.
Immunisations and Vaccinations
Administer childhood immunisations.
Administer adult immunisations.
Deliver seasonal influenza vaccination programmes.
Administer COVID-19 vaccinations (where applicable).
Administer pneumococcal, shingles, RSV and other routine vaccinations.
Deliver travel vaccinations in accordance with current guidance and practice protocols.
Maintain vaccine cold chain requirements.
Ensure vaccine stock is monitored and stored safely.
Work under Patient Group Directions (PGDs) and Patient Specific Directions (PSDs).
Health Promotion and Prevention
Promote healthy lifestyle choices.
Deliver smoking cessation advice.
Support weight management.
Promote physical activity.
Deliver cardiovascular disease prevention advice.
Promote cancer screening uptake.
Encourage vaccination uptake.
Support patients to self-manage long-term conditions.
Signpost patients to local community services where appropriate.
Medicines Management
Administer medications safely under PGDs or PSDs.
Ensure medicines are administered safely and appropriately documented.
Provide patient education regarding prescribed medications.
Identify medication issues requiring GP review.
Support safe medicines management within the practice.
Infection Prevention and Control
Maintain high standards of infection prevention and control.
Prepare and maintain treatment rooms.
Ensure equipment is cleaned and maintained appropriately.
Undertake cleaning schedules where applicable.
Maintain stock levels of clinical consumables.
Participate in infection control audits.
Report infection control concerns promptly.
Quality Improvement and Clinical Governance
Participate in clinical audit.
Contribute to quality improvement projects.
Support achievement of QOF targets.
Support delivery of Enhanced Services.
Participate in significant event reviews where appropriate.
Assist with implementation of new clinical guidance.
Support CQC compliance and inspections.
Identify opportunities to improve patient services.
Safeguarding
Maintain safeguarding knowledge and competencies.
Recognise safeguarding concerns affecting children and adults.
Escalate safeguarding concerns appropriately.
Participate in safeguarding meetings where required.

Code consultations appropriately.
Action pathology results within agreed protocols.
Complete referrals to other services where appropriate.

Qualifications Essential
Registered Nurse (Adult) with current NMC registration. Previous General Practice Nursing experience. Cervical Cytology qualification. Immunisation and Vaccination training. Evidence of Continuing Professional Development (CPD). Current Basic Life Support (BLS) certification. Up-to-date Adult and Children Safeguarding training.
