Job summary
Working closely alongside our GP team, the successful candidate will play a key role in managing acute on-the-day demand and supporting patients within the community.
Due to increasing demand for same day appointments and community-based care, we are seeking an experienced and motivated Advanced Nurse Practitioner (ANP) to join our clinical team for four sessions per week.

Main duties of the job
Key responsibilities will include:
Conducting home visits for housebound and clinically appropriate patients. Managing on-the-day acute presentations. Assessing, diagnosing and treating patients independently within their scope of practice. Supporting and managing triage requests through our newly implemented Klinik online consultation and triage system. Providing clinical advice and signposting patients to the most appropriate service. Prescribing independently. Maintaining accurate and timely clinical records. Working collaboratively with GPs, practice nurses, administrative staff and wider healthcare professionals.

Qualifications Essential
We are looking for an experienced ANP who: Is a registered nurse with current NMC registration. Holds a recognised Advanced Clinical Practice qualification or equivalent experience. Has experience working within general practice, urgent care or primary care settings. Is confident in autonomous assessment, diagnosis and treatment of acute conditions. Has excellent communication and clinical decision-making skills. Is able to effectively manage risk and prioritise workload. Independent prescribing qualification desirable. Experience of digital triage systems, particularly Klinik, would be advantageous although training can be provided.
