Job summary
Coventry & Rugby GP Alliance, in partnership with Dementia UK, is recruiting an experienced registered nurse to join its Admiral Nursing Service as an Admiral Nurse Clinical Lead. This role is ideal for a nurse passionate about relationship-centred dementia care, advocacy, and promoting equality for people living with dementia and their families.
The successful candidate will provide expert one-to-one support, guidance, and practical interventions for families while championing best practice across Coventry and environs. As Clinical Lead, you will manage and supervise the Admiral Nursing team, oversee clinical caseloads, and lead initiatives that enhance dementia care and support from pre-diagnosis through to post-bereavement.
Reporting to the Head of Clinical Patient Services, you will receive ongoing professional support from Dementia UK to strengthen your specialist expertise. You will also join the wider Admiral Nurse network, gaining access to learning opportunities and professional development aligned with the Admiral Nurse Practice Standards Framework.
Applicants must be NMC-registered nurses with substantial post-registration experience supporting people with dementia and their carers across a range of settings, including primary care. A recognised postgraduate qualification or formal clinical training in dementia care is essential.
Candidates are encouraged to complete Dementia UKs free Pre-Admiral Nurse eLearning course before interview or upon appointment.
Main duties of the job
The Admiral Nurse will lead, deliver and coordinate relationship-centred dementia care within operational areas of Coventry & Rugby GP Alliance, in order to improve the experience of carers/families affected by dementia with complex needs.

Main Duties:
Clinical
Deliver regular clinical 1:1 sessions for families affected by dementia who meet the referral criteria within designated areas/ practicesProvide clinical leadership on the care of people with dementia and their carers/families with complex care needs from peri-diagnosis to post bereavement support, to support reduced reliance on GP appointmentsProvide specialist nursing assessment, using the Admiral Nurse Assessment Framework or appropriate tools for carers/families affected by dementia, including positive risk taking, managing distressed behaviours and risk minimisation strategiesProvide a range of bio-psychosocial interventions to promote health and wellbeing for people with dementia and their carers/familiesWork collaboratively with other professionals to improve the identification and management of co morbidities and frailty for people with dementia and their carers/familiesDevelop and support care plans for people with dementia and families including positive risk managementSupport people with dementia, carers/families and staff to understand and respond to changes in behaviour and relationships as a result of dementiaProvide care coordination for a clinical caseload of families with complex needs as identified within triage assessmentEnsure the safeguarding of vulnerable adults and that all legal requirements relating to Mental Capacity Act 2005, Mental Health Act 1983 (amended 2007), and Deprivation of Liberty Safeguards (2009) are considered and adhered to in relation to patient careAct as an advocate for people with dementia and their carers/families to promote their rights and best interestsParticipate and lead on case conferences, continuing health care processes and best interest meetings where requiredWork collaboratively with health, social and voluntary sector services to achieve coordinated careWork in partnership with colleagues to promote the use of advance care planning for people with dementia and to support and inform families, around advance care planning, best interest decisions and end of life careManage and advise on complex care transitions for people with dementia and their carers/families to reduce the frequency of crisis situationsRegularly attend multi-disciplinary team meetings for long-term conditions to inform and receive information regarding patients who have a diagnosis of dementiaRefer to secondary care if there is deterioration in condition/a review of medication including antipsychotics is required or any other concerns regarding a persons health that cannot be managed in primary care, in line with their dementia expertiseSupport and advise on carer assessments and reviewsEnsure that care is delivered in accordance with relevant local and national guidelines and all relevant Coventry & Rugby GP Alliance policiesRecognise the limits of own competency and professional boundaries and make appropriate and timely referrals to other services where requiredEnsure that clinical records are up to date and maintained and stored appropriately in line with local and national GDPR regulations
Education and Leadership
Facilitate and support the development and delivery of innovative dementia training and education to staff, to improve competence and confidence, in line with best practice and guidanceParticipate in and/or lead formal training events promoting best practice in dementia care and disseminating Admiral Nursing workProvide professional and clinical leadership and mentorship, acting as a clinical role model on the delivery of evidence-based practice in dementia care within Coventry & Rugby GP Alliance.Advise and support on the development and delivery of work streams as part of Coventry & Rugby GP Alliances, dementia strategy, where appropriateWork proactively with key local and national stakeholders to develop more integrated care pathways and holistic models of care, through dissemination of specialist skills and knowledgeContribute to the delivery of local policy, procedures and protocol for Coventry & Rugby GP Alliance to improve identification, assessment, ongoing support for people with dementia and their family carers.
Evaluation and Audit
Support the audit and evaluation of quality improvements and initiatives aimed at improving the delivery of dementia care within Coventry & Rugby GP Alliance, where appropriate to roleCollect and review quantitative and qualitative data, as agreed, to evaluate and audit the Admiral Nurse Service and make necessary improvementsUndertake analysis and evaluation and make any recommendations to the Coventry & Rugby GP Alliances Dementia Strategy Group or similarEnsure service information and data recorded is relevant, accurate, complete and captured in a contemporaneous mannerSupport evaluation of workforce development in dementia to improve staff knowledge and skills across Coventry & Rugby GP AllianceLiaise with Dementia UK Insights and Evaluation team where appropriate, to ensure that the impact and effectiveness of Admiral Nurse interventions are well evaluatedBe fully aware of and comply with the Data Protection Act (2018) and the GDPR in relation to protecting the confidentiality of patient and service-user information and enabling appropriate information-sharing.
