Job summary
We are actively seeking candidates to apply for the role of a CYP Coordinator at The Confederation.
There are two vacancies available. Both are full-time on a one year fixed term contract. The role also requires working two evenings per week and one Saturday per month.
The Children & Young People's (CYP) Co-ordinator will support children, young people and their families to access the right mental health and wellbeing support at the right time.
Working across Primary Care Networks, the postholder will act as a central coordinator between general practice, specialist NHS services, local authority teams, education, voluntary sector organisations and community services.
Main duties of the job
The role combines care coordination, service navigation, partnership working, pathway development and quality improvement to improve access to early intervention and reduce unnecessary escalation into crisis services.
The postholder will manage referrals, coordinate care, develop strong relationships across organisations and contribute to service improvement through data collection, reporting and evaluation.

Coordinate referrals across multiple servicesReceive, review and triage referrals into the CYP programmeBuild collaborative relationships across organisationsProvide advocacy, tailored support and empower self-management within sessionsCoordinate onward referrals to the most appropriate servicesSupport children, young people and families to navigate local servicesDevelop individual support plans where appropriateMaintain regular contact with families whilst awaiting supportEscalate safeguarding or clinical concerns appropriatelyMaintain accurate clinical records using digital systemsRepresent the CYP Programme across Hillingdon

Person Specification
Qualifications Essential
Right to Work in the UK Evidence of a high standard of written and spoken English (GCSE or equivalent) Degree or equivalent Level 6 qualification in a relevant subject (e.g. Psychology, Social Work, Mental Health, Public Health, Medical & Ethics, Health & Social Care, Childhood Studies, Nursing or a related discipline).
