This is not a post where you will be handed a list and left to it. Our nursing team works alongside our mult-skilled clinical team to deliver excellent patient care.
Main duties of the job What you will be doing
Nurse-led long-term condition reviews across diabetes, asthma, COPD, hypertension and other LTC reviews.
Cervical screening, childhood and adult immunisations, travel health and NHS Health Checks.
Wound care, dressings, ear irrigation, ECGs, according to your competencies.
Contributing to QOF delivery, recall and call, and to audit and quality improvement.
Supporting infection prevention and control, cold chain and vaccine governance.

Job description Job responsibilities Job purpose
To deliver safe, evidence-based nursing care to the practice population, working autonomously within the limits of NMC registration and agreed personal competence. The post holder will lead nurse-delivered long-term condition management, national screening and immunisation programmes, and treatment room services, and will contribute to the practice's quality improvement, QOF and clinical governance work.
Key responsibilities1 Clinical care
Assess, plan, deliver and evaluate nursing care for patients attending the practice, working within agreed protocols and escalating appropriately.
Deliver structured annual and interim reviews for patients with diabetes, asthma, COPD, hypertension, and other long-term conditions.
Undertake cervical cytology within the NHS Cervical Screening Programme, maintaining the required sample-taker registration and update training.
Deliver childhood and adult immunisation programmes, seasonal vaccination campaigns and travel health advice in line with the Green Book and PGDs.
Provide treatment room services including wound assessment and dressings, suture and clip removal, ear care, ECG recording and phlebotomy as required.
Deliver NHS Health Checks and opportunistic health promotion, including smoking cessation, weight management and alcohol brief intervention.
Recognise the deteriorating or acutely unwell patient and initiate appropriate emergency response, including anaphylaxis management and basic life support.
2 Medicines and clinical safety
Administer medicines and vaccines under Patient Group Directions/Patient Specific Directions, where required.
Maintain vaccine cold chain integrity, stock control and clinical fridge monitoring in line with practice policy.
Act on Central Alerting System and MHRA safety alerts as directed by the practice CAS Lead.
Report incidents, near misses and significant events promptly and participate in the practice's Significant Event Analysis and PSIRF processes.
3 Quality, data and QOF
Record all clinical activity contemporaneously and accurately using agreed SystmOne codes and templates.
Contribute to the delivery of QOF, enhanced services and national screening targets, including proactive recall and call.
Participate in clinical audit and quality improvement, including closed-loop re-audit.
Support the practice's health inequalities work, including CORE20PLUS5 cohort identification and targeted outreach.
4 Safeguarding, IPC and governance
Recognise and act on safeguarding concerns for children and adults at risk, escalating to the practice Safeguarding Lead in line with local multi-agency procedures.
Maintain infection prevention and control standards, including decontamination, sharps safety, waste segregation and IPC audit participation.
Work within the practice's information governance and confidentiality framework, including the Data Security and Protection Toolkit requirements.
5 Team, learning and development
Attend and contribute to clinical meetings, multidisciplinary team discussion and practice learning events.
Engage in scheduled clinical supervision with a named supervisor and maintain the Clinical Competency Validation Matrix record.
Maintain NMC revalidation requirements and a current personal development plan.
Support the induction and development of HCAs, students and new colleagues as appropriate.

Experience Essential
- Minimum 2 years' post-registration clinical experience - Minimum 12 months as a Practice Nurse in general practice, or equivalent recent primary care experience - Demonstrable experience of running nurse-led long-term condition clinics across at least two of diabetes, asthma, COPD, hypertension or CVD - Experience of working autonomously and managing own clinical caseload with minimal supervision - Practical experience of cervical sample taking and immunisation delivery in a live clinic setting - Working understanding of QOF, enhanced services and primary care targets
Desirable
- Participation in clinical audit or quality improvement - Experience supporting HCAs, students or newly appointed colleagues
Qualifications Essential
- Current NMC registration as a Registered Nurse. - Evidence of up-to-date NMC revalidation. - Cervical sample-taker qualification and current update training - Recognised immunisation and vaccination training
Desirable
- Accredited training/update training in diabetes, asthma, COPD or CVD management
Knowledge and skills Essential
- Sound clinical assessment skills and the judgement to work within, and recognise, the limits of own competence - Working knowledge of adult and child safeguarding duties and escalation routes - Understanding of infection prevention and control in a primary care setting - Current BLS and anaphylaxis management competence - Accurate, contemporaneous record keeping and appropriate clinical coding - Understanding of information governance, confidentiality and consent
