About this role

Job summary

The patient pathway coordinator (PPC) is an integral part of the patient pathway for NHS referrals (referral to discharge) ensuring that the process is both efficient and effective. This is achieved by accurately processing and actively monitoring the flow of information throughout the NHS patient pathway.

The PPC will oversee the entire NHS patient pathway for specific specialities from referral to GLH until the point of discharge. They will streamline the patient pathway and optimise patient experience and outcomes, ensuring that clinicians are in possession of all relevant information, to ensure timely, safe and effective care delivery.

In addition to timely and effective triage and scheduling of NHS clinic appointments and procedures, the PPC will ensure timely arrangement of relevant diagnostics and make sure that results from any diagnostics or tests are shared with the relevant consultant/s as well as the patient, the patients GP and relevant NHS Trust, before arranging any subsequent follow up appointments at GLH.

The postholder will have excellent attention to detail and (supported by relevant clinical and operational colleagues), will use initiative and good judgement to problem solve and consider the best course of action to take for NHS patients.

Main duties of the job

Work with clinical colleagues to ensure that NHS activity (clinics, procedures and diagnostics) is arranged safely, effectively and in a timely manner.

For new referrals, review referral information provided by the GP / referring NHS Trust, picking out investigations or tests that have already been completed and ensuring results are available and reviewed in a timely manner.

Ensure that any required diagnostic tests are arranged in a timely manner, taking care to obtain and share results with relevant parties as soon as they are available, escalating any abnormal results immediately.

Ensure results are added to patient record and take necessary follow up actions, including removing patients from waitlists, booking follow up appointments where required, ensuring that all relevant parties (e.g. NHS Trusts are informed of outcomes in a timely manner).

For follow up appointments, ensure these are arranged in a timely manner, ensuring that all relevant diagnostic results are available for review.

Maintain accurate and timely records within the relevant electronic patient record.

Track NHS patient pathways from referral to discharge, highlighting and resolving queries as they are encountered, ensuring that patients, NHS referrers (including NHS Trusts) and consultants are kept fully updated to optimise patient, NHS Organisation and and clinician experience.

Maintain excellent working relationships with internal and external / NHS colleagues, consultants and other providers.

The NHS Patient pathway coordinator is an integral part of the patient pathway (referral to discharge) ensuring that the process is both efficient and effective. This is achieved by accurately processing and actively monitoring the flow of information throughout the patient pathway.

The NHS Patient Pathway Coordinator will ensure that clinicians have all relevant information regarding a patients condition available to them to ensure that patients get the most out of their time with the consultant by ensuring all relevant test results and images are available for review.

The NHS PPC will ensure that results from tests are shared with a patients GP surgery as well as relevant NHS Trust consultant/s. The postholder will have excellent attention to detail and (supported by relevant clinical and operational colleagues) , will use initiative and good judgement to problem solve and consider the best course of action to take for the patient.

The post holder will be expected to undertake duties as determined by the Chief Operating Officer and Head of Clinical Services and those listed under responsibilities with this job description.

Work with clinical colleagues to ensure that NHS activity is effectively prepared for each clinician, including availability of results

For new NHS patient appointments, review referral information provided by the GP / NHS Trust, picking out investigations or tests that have already been completed and ensuring they are available for the consultation

For follow up NHS patient appointments, review the previous clinic letter and referrals out recommended and ensure these have been conducted and the results are available for review.

Process post-clinic documentation by distributing and actioning the contents as required.

Communicate and send results of diagnostic tests to the patients consultant and/or GP within the specified timeframes.

Ensure results are added to patient record and take necessary follow up actions; removing patients from waitlists, booking follow up appointments where required

There may be urgent findings on reports that need actioning immediately, following processes to highlight this to the clinician and follow their guidance on the outcome required to progress the patients care.

Chase reports, results and other documentation working proactively and independently to achieve required outcomes.

Work with the relevant clinical teams and the Bespoke Healthcare radiology manager to manage imaging requests as required

Track NHS patient pathways from referral to discharge, highlighting and resolving queries as they are encountered, ensuring that patients and consultants are kept fully updated to optimise patient and clinician experience.

Maintain excellent working relationships with GLH and NHS colleagues, consultants and external providers.

Maintaining accurate patient records by supporting timely updating of waiting lists, updating tasks, saving delivery / read receipts, adding results and notes to optimise the patient pathway and patient experience.

Manage multiple tasks, prioritising own workload and working with team to ensure work is completed within target timeframes.

Work with GLH data officer and actively contribute to information required for local and national NHS data submissions including referral to treatment (RTT) waiting time data and Diagnostic waiting time targets (DM01).

Undertake any other reasonable duties as commensurate with the nature of this role.

Undertake any mandatory training as appropriate to the post and as directed.

Person Specification

Experience Essential

Good organisational skills and a systematic approach to work with care taken over detail and due regard to confidentiality. Able to work independently and act on own initiative, seeking support and advice as required. Experience of working within a patient / customer focused environment. Self-motivated and resourceful, with the proven ability to multi-task and operate successfully under tight deadlines and time pressures Enjoys a fast pace, dynamic, challenging and team-orientated work environment Computer literate in MS Office e.g. Word, Excel and other window-based programs Strong writing, listening and speaking communication skills Excellent interpersonal and teamwork skills

Qualifications Essential

GCSE Grade A to C in English and Maths
