About this role

Job summary

The General Practice Nurse is responsible for delivering high-quality, evidence-based nursing care to the practice population. The post holder will work autonomously within their professional scope of practice, managing patients with long-term conditions, providing health promotion and disease prevention, delivering treatment room services, and supporting the wider multidisciplinary team.

The role requires excellent clinical judgement, leadership, and communication skills to ensure patients receive safe, effective and compassionate care.

Post is for part-time 16hrs per week

Main duties of the job Clinical Practice

Provide comprehensive nursing care including:

Long-term condition management (Diabetes, Asthma, COPD, Hypertension, CKD and other chronic diseases)

Annual reviews and care planning

NHS Health Checks

Cervical cytology (smear testing)

Childhood and adult immunisations

Travel vaccinations (where appropriately trained)

Wound care and dressings

Removal of sutures and clips

Ear irrigation (where competent)

Venepuncture

ECG recording

Blood pressure monitoring (including ambulatory monitoring)

Urinalysis and specimen collection

Vitamin B12 and other prescribed injections

Contraceptive and sexual health advice within competence

Health promotion and lifestyle advice

Smoking cessation support

Weight management advice

NHS screening programmes

Infection prevention and control procedures

We are a forward thinking, supportive and innovative practice offering a range of services.

As active members of our PCN, the Practice also has access to shared services and resources including Social Prescribers, Clinical Pharmacists, Physiotherapists, MHP, and remote clinics.

The Surgery moved to its new purpose-built home in Jan 2020. We are a friendly, modern practice with a population of approx 8,200 patients.

Main Duties of the Job

The successful applicant will be expected to:

Deliver high-quality nursing care to patients within General Practice.

Undertake chronic disease reviews including Diabetes, Asthma, COPD, Hypertension and other long-term conditions.

Carry out cervical screening (smear tests).

Administer childhood and adult immunisations and vaccinations in accordance with national guidance.

Undertake NHS Health Checks.

Perform venepuncture, ECGs, Ear Syringing

Promote healthy lifestyles through patient education and preventative health interventions.

Assess, plan, implement and evaluate evidence-based nursing care.

Recognise deteriorating patients and escalate concerns appropriately.

Maintain accurate clinical records using SystmOne.

Participate in clinical audit, quality improvement initiatives and QOF delivery.

Work collaboratively with the wider multidisciplinary team to ensure high standards of patient care.

Participate in mandatory training, appraisal and continuing professional development.

Person Specification

Experience Essential

Previous experience working within General Practice or Primary Care.

Qualifications Essential

Registered Adult Nurse with current NMC registration. Previous experience working within General Practice or Primary Care Experience managing long-term conditions. Competent in cervical cytology. Competent in childhood and adult immunisations. Venepuncture and ECG competency. Excellent communication and interpersonal skills. Ability to work independently and prioritise workload. Good IT skills with experience using clinical systems (SystmOne desirable).

Desirable

Diabetes qualification. Respiratory qualification. Travel Health qualification. Experience supporting QOF
