The role of the Pathway Tracker is to provide comprehensive administrative support to the Women's Directorate West, Obstetrics and Gynaecology, by collecting, recording, analysing, interpreting and reporting of data required to monitor compliance with the Welsh Assembly Government Referral to Treatment Time (RTT) and Service and Financial Framework annual targets. Responsible for accurate and timely recording of the whole patient pathway on WPAS. The role involves being an integral member of the Women's Directorate West Management Team, co-ordinating, organising and attending of meetings. The Pathway Tracker reviews the progress the patients journey through the care pathway and core services such as health records, radiology and pathology. This pathway is both within and external to the Organisation and includes liaison with the departments involved in providing care and core services, ensuring that planned patient treatment progresses smoothly and in a timely manner. Proactively takes steps to ensure that the patients diagnostic and treatment pathway are achieved on target

Main duties of the job

Communicates with clinical and non clinical members of the health care team for the benefit of patient care e.g. arranging appointments or referral onto others within or outside the Organisation e.g. other departments in other Organisations.

Must have the skills to question delays in the patients treatment pathway using tact and diplomacy to ensure compliance with pathways and escalation policy

Ensures that the progress of the patient through the pathway is prospectively and proactively managed, any concerns relating to e.g. waiting times and treatment schedules must be communicated to the relevant staff or clinical service and where necessary the post holder will be required to use negotiation and persuasive skills to improve this element of the pathway

Deal with telephone, personal and other enquiries in a timely, professional and effective manner, enquiries may be from clinical departments, health care professionals or patients

To take personal responsibility for administration coordination and sequencing of the clinical pathway for a range of patients (possibly around 200 patients at any one time), at various stages in the hospital journey

Effective communication requires verbal and non verbal skills e.g. email, information systems, communicating directly to clinical/non-clinical colleagues regarding investigation and other appointments

The ability to speak Welsh is desirable for this post; Welsh and /or English speakers are equally welcome to apply.

Qualifications and/or Knowledge Essential
