About this role
1.提供長者日常生活照顧，血壓量測、進食、飲水與如廁協助、沐浴協助、交通車抵達時協助長輩上下車接送與安全維護 2.與社工師、營養師與物理治療師討論，落實於長輩個別化照顧計畫 3.帶領長者團體活動，體能或肌力、懷舊、認知促進、團康遊戲等 4.完成活動紀錄，定期與團隊分享討論。 5.協助場域清潔衛生 參與機構辦理相關之教育訓練以及執行其他主管交辦事項
