About this role
一、為失能者擬定照顧服務計畫及連結或提供長照服務。依「長期照顧(照顧服務、專業服務、交通接送服務、輔具服務及居家無障礙)給付及支付基準」，提供組合編號 AA01「照顧計畫擬定與服務連結」、AA02「照顧管理」。 二、透過家訪及電訪長照2.0服務個案，進行計畫擬定及資源連結與追蹤。 三、方案相關行政庶務（核銷另有行政人員協助主責）。
