About this role

Job summary

We are seeking to recruit an experienced and motivated Practice Nurse to join our team at Runnymede Medical Practice working 20-30 hours per week.

You will be joining an established team of 5 Nurses, Nurse Associates, Healthcare Assistants and Phlebotomists delivering care for our 12,000 patients.

We would welcome applications from individuals with any chronic disease specialism and those with, or wishing to develop, leadership experience.

Runnymede Medical Practice (RMP) is a well-established, forward-thinking four partner GP partnership serving patients across Old Windsor, Englefield Green and Egham.

We are a two-site training practice and have a multidisciplinary team of GPs, paramedics, nurses, phlebotomists, clinical pharmacists, musculoskeletal practitioners, mental health practitioners, social prescribers, care coordinators, and an experienced patient services team.

Wound careInjectionsChildhood ImmunisationsTravel consultations / injectionsCervical CytologyBlood PressureINRsHealth checksHealth promotionSupport in the long-term condition management within the Practice, including being involved with quality improvement and assurance initiativesTeach and advise patients and their families on how to manage their conditionLead on public health campaigns e.g. flu programmeContribute to Practice targets (QOF etc), complying with relevant guidance

Support in the delivery of enhanced serviced and other service requirements

Other Responsibilities;

Support the Infection Prevention Control (IPC) lead and work to the IPC PolicyDevelop, implement and embed an effective care management programme for the frail and housebound patients registered with the organisationEmbed population health management approaches within general practice, targeting health inequalities and improving accessSupport new-to-practice nurses and other healthcare professionals within the Practice

Chaperone patients where necessarySupport the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation

Contribute to and embrace the spectrum of clinical governance

Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children

Recognise the boundaries of their practice and know when and to whom patients should be referred

Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near-miss events

Work within national and local protocols

Participate in clinical audits and research projects and implements changes as required, including supporting with the development, and updating of practice protocols/guidelines and procedures locally

Support public health and screening programmes, including immunisations, vaccinations, and cervical screening and also provide opportunistic health promotion advice
