We are seeking an enthusiastic and proactive Clinical Pharmacist to join our friendly, supportive and forward-thinking GP practice. This is an exciting opportunity to become an integral member of our multidisciplinary team, helping to deliver high-quality, safe and effective patient care.
We would particularly welcome applications from pharmacists who hold an Independent Prescribing qualification, although we are also keen to hear from experienced Clinical Pharmacists who are interested in developing into this role.
Main duties of the job
As our Clinical Pharmacist, you will work closely with GPs, nurses and the wider practice team to optimise medicines management, improve prescribing quality and support patients with long-term conditions.
Your responsibilities will include:
Conducting structured medication reviews. Managing prescribing for patients with long-term conditions. Supporting medicines optimisation and cost-effective prescribing. Responding to medication queries from patients and clinicians. Reviewing hospital discharge summaries and medicines reconciliation. Managing repeat prescribing processes. Undertaking clinical audits and quality improvement initiatives. Supporting the implementation of national and local prescribing guidance. Providing education and advice to the clinical team.
For candidates who are Independent Prescribers, there will also be opportunities to manage your own clinical caseload and run specialist clinics appropriate to your experience.

Water Meadow Surgery is a well-established thriving teaching practice (2 trainers) looking for a motivated clinical pharmacist to providing high-quality services to a varied population as part of our friendly team serving 13,500 patients.
In addition to our doctors and nurses, we have a remote clinical pharmacist (30 hours), a first contact practitioner (4 sessions) a paramedic (8 sessions) and a physician assistant (6 sessions)
Water Meadow Surgery is in the leafy Chilterns, an AONB, at the end of the Metropolitan Line into Central London. We are also close to the Chiltern Line (Amersham 3 miles), direct to Marylebone.

Job Summary
The Clinical Pharmacist will work as part of the multidisciplinary general practice team to improve the quality, safety and cost-effectiveness of prescribing and medicines optimisation. The postholder will undertake medication reviews, support patients with long-term conditions, provide specialist pharmaceutical advice, and contribute to delivering high-quality, evidence-based patient care.
Where appropriately qualified, the postholder will independently assess, diagnose and prescribe within their scope of competence.
Key Responsibilities
Clinical Responsibilities
Undertake structured medication reviews. Optimise medicines for patients with long-term conditions. Identify and reduce medicines-related risks. Review polypharmacy in complex patients. Support care home prescribing. Undertake medicines reconciliation following hospital discharge. Provide pharmaceutical advice to clinicians and patients. Manage medication queries from patients and healthcare professionals. Support safe repeat prescribing processes. Promote antimicrobial stewardship. Assist with medicines safety alerts and recalls. Independent Prescribing (where qualified)
Conduct patient consultations. Assess, diagnose and prescribe within scope of competence. Manage long-term condition clinics. Monitor treatment effectiveness. Refer appropriately when necessary. Medicines Optimisation
Improve prescribing quality using evidence-based guidance. Support implementation of NICE guidance and local prescribing policies. Participate in clinical audit and quality improvement projects. Review prescribing data and identify opportunities for improvement. Support achievement of Quality and Outcomes Framework (QOF) and other contractual requirements. Practice Support
Advise clinicians on prescribing issues. Develop prescribing protocols and guidelines. Deliver educational sessions to practice staff. Contribute to multidisciplinary team meetings. Work collaboratively with community pharmacy, secondary care and other healthcare providers. Administration
Maintain accurate contemporaneous clinical records. Ensure prescribing documentation meets professional standards. Participate in practice meetings. Assist in developing pharmacy services.
Person Specification
Qualifications Essential
MPharm degree or recognised equivalent Current registration with the GPhC Primary Care Pharmacy Education Pathway completion
Desirable
Independent Prescriber qualification. Primary Care Pharmacy Education Pathway completion. Experience working in general practice, PCN or primary care. Experience using SystmOne. Experience managing long-term conditions. Knowledge of QOF, medicines optimisation and NHS prescribing guidance. Experience of clinical audit and quality improvement.
