About this role

Job summary

This is an exciting opportunity to support the development and growth of a continence service within the Acute sector, bridging the gap between Acute and community care with our patients at the heart of the service.

The post holder will work as a specialist nurse within Tissue Viability and Continence team, assessing patients with complex continence needs, providing expert knowledge and clinical care to patients within the acute setting.

Deliver specialist skills in relation to triage, assessment, treatment and management of bladder

and bowel dysfunction.

Provide specialist advice, education and support to health care professionals, and

members of the general public.

Support and implement quality improvement initiatives alongside the lead for Tissue Viability and Continence and the lead for the Specialist pelvic heath team.

Main duties of the job

Act as the key worker and manage a caseload of inpatients as appropriate within the specialty, delivering individualised and personalised direct patient care

Provide clinical expertise and specialist advice across the whole spectrum of the service. Develop and maintain the competencies and skills needed to practice effectively ensuring up to date clinical Continence knowledge on key developments, interpreting and applying knowledge to practice through education and consultation with colleagues and patients. Undertake continued professional development and maintain a written record

Job description Job responsibilities

Demonstrate in depth specialist knowledge, skills and innovative practice in continence management ensuring the delivery of appropriate care

Provide clinical expertise and specialist advice across the whole spectrum of the service.

Promote the philosophy of patient centred care and establish and maintain a supportive relationship with the patient and their family during the period of treatment and care while in hospital referring on to community services once discharged.

In conjunction with the multi-disciplinary team (MDT) and as a core member of the MDT, monitor effectiveness of planned care and treatment, advising on changes in care management.

Promote high quality clinical care, ensuring the use of evidence-based practice and clinical standards, and to ensure that local guidelines/policies are disseminated and adopted within the specialty.

Demonstrate appropriate expert practice and specialist advice in the assessment, planning, implementation and evaluation of patient care. This will necessitate high levels of autonomy and advocacy. Use expertise and decision-making skills to inform clinical reasoning approaches when dealing with differentiated and undifferentiated individual presentations and complex situations, synthesising information from multiple sources to make appropriate, evidence-based judgements and/or diagnoses. Provide specialist education to all members of the clinical team, actively encouraging lifelong learning and personal development.

Lead the development of a patient focused education and information giving and provide training for patients and their families to self-manage continence issues.
